Job responsibilities

Main duties of the job:

Providing on the day mental health coaching for patients one to one, via phone call or face to face, within GP practices, and follow up appointments to ensure Patients feel support and have access to all appropriate care pathways

Provide mental health risk assessments for patients, ensuring they are not in any immediate danger to themselves or others

Help Patients gain the knowledge, skills and confidence to become active participants in their care so that they can reach their self- determined health and wellbeing goals, and improve levels of patient activation.

Raise awareness and increases responsibility for mental health and wellbeing and supporting Patients to change their relationship to a problem or challenge

Use strong communication and negotiation skills to outline potential risks and consequences and to ensure that people understand the implications and accountability of making their own decisions around healthcare.

Connect Patients to community-based activities or local health, social care and voluntary services to further support their health care plan or refer to a Social Prescriber within the Primary Care Network.

Working with others in the PCN practices to undertake NHS health checks in line with protocols.

To maintain accurate clinical records of all patient consultations and related work.

To promote mental health coaching amongst the PCN and practices and to support colleagues to improve their skills and understanding of personalised care.

To liaise with other health and social care professionals and agencies to ensure holistic care is provided and the care packages offered are appropriate and consistent with the patients needs and aspirations.

Produce complete and accurate records of patient consultation, in line with best practice, confidentiality, policies and procedures

Deliver care according to NHS guidance, NICE guidelines and evidence-based care

Manage, review and identify learning from patient complaints, clinical incidents and near-miss events

Awareness of statutory safeguarding, notification processes and local guidance for children/vulnerable patients, applying relevant policies and legislation to protect them

Ensure compliance with policies, procedures and guidelines for self and others, by taking action or alerting senior management team if the practice appears to contravene policy, or if there are concerns over any aspect of patient care.

Experience

Essential

• Knowledge of the Personalised care approach.
• Understanding of the determinants of health to include social, economic and environmental factors.
• Knowledge of the needs of patients with short term and long term mental health conditions.
• Understanding of, and commitment to, equality, diversity and inclusion.
• Knowledge of IT systems to include the ability to use word processing skills, emails and internet.
• Ability to build and maintain relationships whilst maintaining appropriate professional boundaries.
• Excellent verbal communication skills with the ability to communicate effectively at all levels including patients and carers, specialist services, GPs and colleagues. Listening skills displaying empathy.
• Ability to support and mentor staff and volunteers.
• Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences.
• Good technical literacy of Microsoft Applications e.g. Word, Excel, and Access etc.

Desirable

• Experience of working in Mental Health and Primary Care.
• Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement (can include unpaid work).
• Experience of supporting people, their families and carers in a related role (can include unpaid work).
• Experience of supporting people with their mental health, either in a paid, unpaid or informal capacity.
• Experience of data collection and using tools to measure the impact of services.
• Knowledge of community development approaches.
• Knowledge of public health issues in the local area.
• Knowledge and experience of training volunteers.
• Experience of partnership/collaborative working and of building relationships across a variety of organisations including the voluntary sector.
• Experience of using SystmOne.

Qualifications

Essential

• Educated to GCSE or equivalent (GCSE Maths & English C or above).
• Demonstrable commitment to professional and personal development.
• Training in motivational coaching, behavioural change and goal setting.

Desirable

• Health and Fitness qualification L3,
• Coaching Qualification(s) / Mentoring L3,
• Mental Health First Aid
