Oakhaven Hospice is seeking a Community Nurse to join our team on a fixed-term basis to cover maternity leave. Youll support patients and families in the comfort of their own homes during this vital time.

Main duties of the job

This maternity cover role offers a unique opportunity to work as part of our experienced Community Team, providing high-quality, patient-centred palliative care in the community. You'll play a key part in ensuring continuity of care and support for patients and their loved ones.

Assess, plan, implement, and evaluate individualised nursing care for patients with complex needs.

Provide emotional and practical support to families and carers.

Work collaboratively with GPs, district nurses, and other healthcare professionals.

Contribute to 24/7 community care by participating in a rota that includes some evenings/weekends.

Maintain accurate records and uphold the highest standards of clinical governance.

What Were Looking For

Registered Nurse (NMC Registered).

Experience in palliative care, oncology, community nursing, (training can be provided).

Experience is required of caring for families and patients facing the challenge of end of life.

Excellent communication skills to enable you to discuss difficult conversations

Compassionate manner and a responsible, mature attitude are also essential

Good organisation skills and be IT literate as well as being a great problem solver with the ability to think creatively.

Able to work autonomously and as part of a team.

A full UK driving licence and access to a car.

  • Registered General Nurse.
  • First-level degree or demonstrate equivalent experience and skill
  • Post-registration qualification in cancer/palliative care or relevant subject or working towards
  • Evidence of continued professional development

Desirable

  • Teaching qualification

  • Relevant post registration experience in cancer, palliative care, or another speciality.
