Main duties of the job

As the Lead for Community Nursing, you will play a pivotal role in shaping and delivering our community services. Working closely with patients, families, GPs, and wider healthcare professionals, you will coordinate care that respects dignity, autonomy, and individual choice. You will also support and inspire your team to achieve clinical excellence and innovation.

  • Your leadership will ensure our services are responsive, efficient, and aligned with the needs of our local community
  • Provide strategic and operational leadership to the Community Nursing Team.
  • Lead the delivery of high-quality palliative care within patients homes.
  • Collaborate with internal and external partners to promote integrated care.
  • Support the education, development, and wellbeing of community nursing staff.
  • Monitor service effectiveness and contribute to continuous quality improvement.

We are looking for someone with

Registered Nurse with current NMC registration and a First Level degree in a related subject.

Significant experience in community nursing and palliative/end-of-life care.

Proven leadership or management experience within a healthcare setting.

Excellent communication, coordination, and team-building skills.

A passion for holistic, patient-centred care in line with hospice values

Essential

  • Minimum of 5 years relevant post registration experience, at Band 6 or above, in cancer, palliative care/Hospice or speciality.
  • Evidence of multi-professional working
  • Evidence of operational service management and leadership at a senior level in a relevant environment, including people and finance.
  • Demonstrable previous success in leading and delivering change and performance with and through clinical teams, by engaging them in the strategic direction and delivery plans, establishing clear work priorities with them, delegating effectively, ensuring a capability to deliver, monitoring performance and giving feedback.
  • Proven ability to analyse complex problems and to develop and successfully implement practical and workable solutions to address them. Ability to think and plan strategically, tactically and creatively, and to prioritise work programs in the face of competing demands.
  • Evidence of leading a change management initiative which improved patient care
  • Experience of teaching and developing staff both formally and informally.
  • Evidence of good communications skills
  • Evidence of Line Management including managing employment relation issues, recruitment of staff and staff development
