We are looking for an experienced nurse to lead and develop our dedicated Community Nursing Team, ensuring the highest standards of palliative and end-of-life care for patients in their homes
Main duties of the job
As the Lead for Community Nursing, you will play a pivotal role in shaping and delivering our community services. Working closely with patients, families, GPs, and wider healthcare professionals, you will coordinate care that respects dignity, autonomy, and individual choice. You will also support and inspire your team to achieve clinical excellence and innovation.
- Your leadership will ensure our services are responsive, efficient, and aligned with the needs of our local community
- Provide strategic and operational leadership to the Community Nursing Team.
- Lead the delivery of high-quality palliative care within patients homes.
- Collaborate with internal and external partners to promote integrated care.
- Support the education, development, and wellbeing of community nursing staff.
- Monitor service effectiveness and contribute to continuous quality improvement.
