Job summary
We are looking for a compassionate and committed Senior Practice Nurse/Clinical Team Leader to join our team at Surrey Heath Community Providers. The role will involve providing guidance and support across SHCPL and the Primary Care Network (PCN) providing clinical leadership, team management, helping to set standards, develop clinical pathways and share good practice and learning.
Main duties of the job
Role Summary:
- To provide guidance and support across SHCPL and the Primary Care Network (PCN) providing clinical appointments, clinical leadership, team management, helping to set standards, develop clinical pathways and share good practice and learning.
- To work clinically with an agreed clinical work plan [agreed number of clinical sessions], in line with the scope of your professional practice.
- To support the Clinical Services Manager with service delivery and service improvement, including taking a lead on development of Standard Operating Procedures and clinical policies, and taking a lead on quality improvement initiatives.
- To provide comprehensive line management, support and supervision to a defined team of clinical and patient-facing individuals and teams.
- To take a lead on the provision of clinical services from the SHCPL / PCN clinical Hub, including day to day operations, Infection Prevention & Control, clinical compliance, clinical audit and patient safety.
- To provide operational leadership to the rotad clinical team and clinical activity within the SHCPL / PCN Hub.
- To undertake project work and evaluations linked to development, quality and governance
