Job summary

We are looking to recruit an enthusiastic Band 4 Proactive Care Assistant to join the Proactive Care Team based at Theta Building in Frimley. This is an exciting opportunity for a highly motivated individual, to learn or gain experience in therapy and to join a supportive and friendly team.

The Proactive Care Assistant will have a hybrid role with Administration and Therapy Assistant responsibilities. We anticipate this role will be 20% administrative duties and 80% therapy assistant but this is subject to review as the role becomes established.

This is an evolving and exciting role and therefore the post holder should demonstrate a flexible and positive attitude, to change with the needs of the service and service users.

Main duties of the job

Proactive Care is an NHS Long Term Plan that aims to provide proactive and personalised support for a targeted subset of individuals living with multiple long-term conditions. This service supports adult patients and carers with complex needs who could benefit from proactive Multi-Disciplinary Teams (MDTs) in local communities to be able to live independently for longer and to reduce avoidable hospital admissions and GP visits.

The Proactive Care Assistant will work under the supervision of the Occupational Therapists to proactively case find and identify patients who are most vulnerable from admission and/or health deterioration and work closely with the therapy team to support patients to manage their health conditions and to achieve the best quality of life and independence possible through robust health assessments and care planning.

Essential

  • 4 GCSEs (C and above) or equivalent.

Desirable

  • Relevant NVQ Level 3 qualification or equivalent.

Knowledge

Essential

  • Experience of working with healthcare professionals and/or previous experience in the NHS or social care.
  • Experience coordinating with multiple stakeholder or individuals to meet specified outcomes.
  • Experience providing advice/signposting.
  • Experience using a patient clinical system such as EMIS.
