work as a palliative care nurse within the KICS team for The Kirkwood.
This is an opportunity to work collaboratively with
other healthcare professionals, develop your skills and gain knowledge and
experience in community palliative care working alongside a supportive and
innovative team whilst developing specialist palliative care skills as part of
our in-patient team.
Main duties of the job
Key Responsibilities:
Provide expert nursing care and symptom management
for patients with palliative needs.
Provide holistic assessment and ongoing support to
patients and families throughout their journey.
Work collaboratively with GPs, district nurses and
other healthcare professionals.
Advocate for patients wishes and ensure dignity
and comfort at all stages of care.
Participate in multidisciplinary meetings and
contribute to care and discharge planning and coordination of care needs
Rotate onto the in-patient unit, working as part of
our inter disciplinary team, supporting the delivery of specialist palliative
care in an in-patient setting.
About You:
Registered Nurse (NMC) with experience in
palliative or end-of-life care.
Excellent communication and interpersonal skills.
Ability to work independently and manage a
caseload.
A compassionate, empathetic approach to care.
Full UK driving license and access to a vehicle
(for community visits).
Experience of palliative care and an interest in
further developing specialist skills

Work with patients and carers in a holistic way, addressing physical, psychological, emotional, behavioural, spiritual, and social needs.
Work in partnership with patients, their families, and carers, to enable them to make informed choices; set goals; and toidentifyand promote strategies to adapt to life-threatening and life-limiting situations through effective Advance Care Planning (ACP). Facilitate these discussions, providing information andassistingwith completion of documentationrequired.
Establish andmaintaineffective communication with patients, carers/relatives and professionals across health and socialservices,.Utilise advanced communication, negotiation and diplomacy skills and break down any barriers. Use interpreters and other communication aids asrequired.
