Job summary

An exciting opportunity has arisen to join The Kirkwood as a Specialist Palliative Care Team Manager. The post holderwill carry continuing responsibility for the effective and efficient management of The Kirkwoods Specialist palliative care team, providing management and professional leadership of the nursing team.

Main duties of the job

The Unit Manager will also focus on patient and service user involvement to ensure services are inclusive and codesigned with patients, families and staff to ensure services meet the needs of patients across all communities in Kirklees. The current service comprises of 3 caseloads across the area of Kirklees. The postholder will be directly responsible for the management of the team of specialist palliative care community nurses, as well as the departments clinical budget and effective use of its resources. As part of the multi-disciplinary team the postholder will have regular support from our consultant led medical team.

The ideal candidate will be a first level registered nurse, with specialist palliative care knowledge gained through further education and experience, hold a supporting learners in practice (SLIP) or equivalent teaching qualification and assessing qualification and have a non-medical prescriber qualification or willingness to work towards it, with extensive experience of palliative care at NHS AfC band 6 or 7 or equivalent with experience of leading and managing a team, teaching and mentoring, recruitment and retention and effective resource management.

The postholder should have excellent communication, interpersonal and organisational skills, be emotionally resilient, be committed to continuous professional development, and hold a full UK driving licence with access to a vehicle for work purposes.

Job responsibilities

The Specialist Palliative Care Team Manager will carry continuing responsibility for the effective and efficient management of The Kirkwoods Specialist palliative care team, providing management and professional leadership of the nursing team.

A key element of this role requires the personalising of individual patient care based upon the needs and wishes of the patient and their family, support by the wider MDT in delivering this service. The post-holder will also be directly involved in succession planning of existing staff.

The post-holder is responsible for ensuring the assessment of care needs and that the development, implementation and evaluation of programmes of care are maintained to a high standard. You will support and empower staff to lead and develop services and promote a multi-professional approach with shared care and accountability, with the aim of further developing models of care across the hospice setting that support delivery of the highest standards of personalised patient care.

There will be a focus on the closer integration of the Community clinical nurse specialists and the inpatient unit to support continuity of care and a sharing of skills and knowledge. The

Specialist palliative care Manager will ensure cost effective use of resources, managing the administrative and financial aspects of the team.

The Specialist palliative care Manager will also focus on patient and service user involvement to ensure services are inclusive and codesigned with patients, families and staff to ensure services meet the needs of patients across all communities in Kirklees.

JOB DIMENSIONS

Current service comprises of 3 caseloads across the area of Kirklees, incorporating an advice and support line 24 hours a day which supports admissions and advice to patients and carers in their own home. The role requires direct management of the team of specialist palliative care community nurses. The jobholder is also responsible for the department clinical budget and effective use of resources.

PRINCIPLE DUTIES AND AREAS OF RESPONSIBILITY

Clinical

Provide a clinical lead and role model to the nursing team by delivering high quality care as an expert practitioner.

Set, monitor and seek to continuously improve standards and the quality of patient care utilising benchmarks.

Maintain a safe working environment.

Supervise and maintain accurate records in accordance with NMC guidance and auditing standards.

Ensure that all nursing staff follow current and up to date safeguarding practices including MCA and best interest practice and decision making.

Support and inspire all members of the Multidisciplinary team in its widest sense and ensuring development of advanced/specialist palliative knowledge skills and competent professional practice.

Work collaboratively with the Multidisciplinary team promoting and utilising a leadership approach to empower and ensure shared accountability for shared clinical decisions.

To effectively engage and involve patient, families and carers in its widest sense and at every opportunity encouraging a co design approach to service development.

Ensure the safe use and efficient management of equipment and supplies.

Promote evidence-based practice, wherever possible.

Work with the Matron to ensure that clinical governance processes at unit/dept level encompass measures to ensure the quality of the fundamental and essential aspects of care.

Support the implementation of benchmarking.

Prioritise their workload and that of the wider team within agreed objectives deciding when to refer to others as appropriate.

Acts in accordance with the Nursing & Midwifery code of conduct.

Ensure management of clinical services and efficient redeployment of staff according to service needs.

Education and Training

Ensure all staff have an individual performance development plan.

Ensure all nursing staff maintain mandatory training requirement.

Ensure implementation of the Hospices Study Leave policy.

Ensure the induction and development of staff.

Promote a positive learning environment within the clinical area.

Maintain detailed and accurate records regarding staff education and attendance management.

Actively participate in the palliative care education programme provided by the hospice.

Leadership/Managerial

Provide leadership to the clinical team and working with Matron, develop new roles/ways of working.

Delegate and empower staff without abdicating overall, continuing responsibility.

Line manages the nursing team, including recruitment, performance management and workforce planning.

Supervise staff rostering, ensuring adequate skill mix.

Monitor the use of bank and agency.

Investigate and respond to accidents, complaints, untoward incidents and other significant events whilst allocating responsibility to ensure these are managed effectively and all learning is shared.

Work with Matron to promote the development of all staff.

Deliver an effective quality service within agreed budget.

Deputise for Matron in their absence.

Monitor and evaluate through clinical audit and formal evaluation the standard of care provided by the Palliative Care team.
