Job summary

An exciting opportunity has arisen to recruit a Paramedic to join our multi-disciplinary team working at Crayford Town Surgery, one of the practices in the North Bexley PCN. Are you an experienced Paramedic who is keen to work within primary care with a passion to deliver an excellent service within general practice? If so, then we want to hear from you.

This role plays a pivotal part in driving forward the vision of a wider community service providing the first point of contact for patients presenting with undifferentiated, undiagnosed problems relating to minor illness or injury, abdominal pains, chest pains, etc. so enhancing and improving the quality of care for the most vulnerable patients across the PCN.

The Paramedic will be expected to work autonomously within primary care at an advanced level of practice, using their enhanced clinical assessment and treatment skills and will have a key role in supporting delivery of the Network Contract DES Service Specifications.

We offer a variety of opportunities including peer support and shared learning to enhance clinical competencies and enable continuous development in the role and support ongoing professional development.

The role is offered on a full-time (37.5 hrs per week) basis or part-time may be considered. The appointment will be made initially on a Fixed Term Contract to March 2024. It is highly likely that serious consideration will be given to extending the contract or making it permanent.

Main duties of the job

Working as an autonomous, accountable Paramedic at an advanced level of practice, as part of a multi-disciplinary team (MDT) to assess and triage patients and provide definitive treatment as appropriate.

Assess, diagnose, treat, refer or signpost patients who attend surgery with undifferentiated or undiagnosed conditions relating to minor illness or minor injury.

Promote and empower patients to make informed choices about their treatment using advanced clinical skills.

    Job description

    Job responsibilities

    • Work as an autonomous, accountable Paramedic, as part of a multi-disciplinary team (MDT) within the PCN.
    • Assess and triage patients, including same day triage, and as appropriate provide definitive treatment and make necessary referrals to other members of the primary health care team. (Patients with problems needing referral to secondary care should be discussed with the registered GP if possible, before making such referral).
    • Assess, diagnose, treat, refer or signpost patients who attend surgery with undifferentiated or undiagnosed conditions relating to minor illness or minor injury.
    • Ensure clinical practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations.
    • Use advanced clinical skills to provide education to patients, promoting self-care and empowering them to make informed choices about their treatment.
    • Promote self-management where appropriate including signposting patients to social prescribing and other community and voluntary services.
    • Be able to:
    1. Perform specialist health checks and reviews
    2. Perform and interpret ECGs; alongside other results as appropriate
    3. Perform investigatory procedures, as required
    4. Undertake the collection of pathological specimens including intravenous blood samples, swabs, etc.

    • Support the delivery of anticipatory care and lead certain community services (e.g. monitoring blood pressures and diabetes risk of elderly patients living in sheltered housing).
    • Provide an alternative model to urgent and same day GP home visit for the PCN and undertake clinical audits.
    • Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required and accepting referrals and referring to specialist services, where appropriate.
    • Communicate at all levels across PCNs and other organisations, ensuring effective, patient-centred service.
    • Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice.
    • Communicate effectively with patients, and where appropriate family members and their carers, where applicable, complex and sensitive information regarding their physical health needs, results, findings and treatment choices.
    • Assess own performance and take accountability for own actions and development, either directly or under supervision.

    • Experienced paramedic with excellent clinical competence.

    • Safeguarding and other mandatory training.
    • Basic life support training.

    • Specialist knowledge acquired from post-graduate diploma level / equivalent training/experience.
    • Working towards developing Level 7 capability in paramedic areas of practice and, within six months of the commencement complete the clinical pillar competencies of the Advanced Clinical Practice Framework.
