Job summary

Acorns Childrens Hospice are now recruiting for the exciting new role of Clinical Outreach Nurse to join us in our Walsall, Worcester or Selly Oak Hospice, as part of the expansion of our outreach team. The Clinical Outreach Nurse will deliver clinical care to life limited children requiring short breaks in their own homes. This role is offered on a part-time basis, working 3 days per week and a 12-month fixed term contract.

Main duties of the job

As Clinical Outreach Nurse, you will provide quality clinical care to children who are eligible to access our services, but who are unable to attend the hospice. Youll undertake assessment of clinical needs of children and create care plans and associate risk assessments to deliver the support required. The Clinical Outreach Nurse will work as part of a small outreach team working alongside our Lead Nurse for Clinical Outreach and Clinical Support Worker.

Youll work in consultation with all relevant senior care managers to identify and respond to opportunities for outreach work.

Job description

Job responsibilities

1. In consultation with all relevant senior care managers identify and respond to opportunities for outreach work.

2. Undertake assessment of clinical needs of children identified for outreach care and create care plans and associated risk assessments to deliver the support required.

3. As part of the clinical nursing team work together with the Care Education team, to ensure you have the relevant competencies and enable training to support safe and effective clinical care.

4. Ensure effective communication between the family, Acorns employees and members of the wider multi-professional teams involved in the childs care.

5. Ensure communications are supported by robust documentation.

6. Support planning and provision of scheduled visits by appropriate and competent employee.

7. Work with the Lead nurse for Clinical Outreach and the Clinical Support Worker to maximise the use of resources to provide safe effective care for children in their home or other appropriate environment

8. Attend multidisciplinary meetings to ensure Acorns is represented e.g. Discharge planning, network meetings, etc., where relevant.

9. Make effective use of digital systems to record and inform care delivery ensuring all activity is recorded in the childs electronic record.

10. Participate in service evaluation and undertake audit activity as directed.

11. Use evidence to underpin practice by adhering to Acorns policies and procedures maintaining an awareness of new and developing care options.

12. Maintain an up-to-date knowledge and awareness of issues required to deliver quality care at the end of life to provide effective delivery of care and support to children and their families.

13. Maintain an up-to-date knowledge and awareness of issues around care of children and young people with complex needs including LTV, TPN and Central Lines and any other complex care required in order to provide effective care delivery.

14. Maintain an up to date knowledge and awareness of issues around the safeguarding of children, and to ensure that appropriate steps are facilitated through effective communication across multidisciplinary agencies.

15. Maintain an awareness of the diversity of families supported by Acorns and ensure that the cultural needs of the children and families are met appropriately.

16. Ensure professional boundaries are maintained and that due attention is paid to confidentiality, privacy and dignity at all times.

17. Take advantage of training opportunities to further develop your skills and expertise to enhance your role within the Acorns Care Team including mandatory training sessions.

18. Maintain professional registration and work within the NMC Code of Conduct.

19. Inform the Lead for Clinical Outreach and / or Matron of incidents and concerns using agreed processes.

20. Access regular clinical supervision.

21. Provide case supervision as required to colleagues delivering outreach care.

22. Carry out any reasonable additional duty as agreed by the Lead Nurse for Outreach to meet the changing requirements of the service.

23. Attend team meetings and team briefings and participate in other meetings as required.

24. Ensure that infection control guidelines and policies are strictly adhered to.

25. Be aware of and work within the lone working policies of the organisation.

  • RSCN/RN (Child).
  • Paediatric Palliative Care Qualification or willing to work towards.
  • Care and Assessment of the acutely sick child or willing to work towards.

Desirable

  • ENB 998 or equivalent teaching qualification.
  • Bereavement and Loss Counselling.

Experience

Essential

  • Registered paediatric nurse with experience of acute and community care including support of children with complex and technological care needs.
  • Experience of supporting children and families at the end of life.
  • Experience of safeguarding.
  • Experience of contributing to Multi-disciplinary meetings
