Job summary

Join St Lukes Hospice as a Band E Community Nurse and deliver expert palliative and end-of-life care in the community. You will work within a multidisciplinary team, providing holistic assessments, symptom management, and advanced care planning. The role involves supporting patients and families with dignity and choice, mentoring junior colleagues, and contributing to training and service development. This is a permanent, full-time position based at Little Common Lane, offering a supportive, inclusive environment where you can make a real difference in peoples lives.

Main duties of the job

Provide patient-centred palliative and end-of-life care, including assessment, symptom management, and advanced care planning. Work closely with a multidisciplinary team to reduce avoidable hospital admissions and support patient choice. Mentor junior staff and students, participate in training, audits, and reflective practice, and contribute to service development. Maintain professional standards, represent St Lukes positively, and ensure care is delivered safely, effectively, and compassionately across the community.

  • Current Registration on Part 1 of the NMC/ HCPC register
  • First level Degree in a health care subject or a willingness to undertake within a defined time period
  • Teaching and Assessing Qualification Mentorship qualification
  • Advanced assessment skills (APACS)

Desirable

  • Advanced Communication Skills Training (NCAT) or a willingness to undertake
  • Non-medical prescribing
