An excellent opportunity as arisen for an enthusiastic and highly motivated Nurse to join our team at Halton Haven Hospice.
HaltonHaven is a small and friendly hospice, much loved by our community, serving people from Halton and West Cheshire.We are a healthcare charity that delivers vital care, support, and advice to people suffering from life limiting illnesses and their families, every day of the year. We employ around 60 staff.
The ideal candidate will provide high quality person-centered nursing care in an adult hospice setting. Duties will include administering medication and treatments, providing a plan of care to support the patient with symptom management or end of life care.
The role will involve ensuring patients are supported, to maintain privacy and dignity within their individual medical, spiritual and emotional needs, whilst also providing excellent end of life care, acting as a patient advocate where necessary.
Support clinical team members by working collaboratively to deliver high quality, palliative end of life care.Palliative experience and end of life care are desirable.
The successful post holder with be professional with excellent engagement and communication skills.The postholder will be organised, able to prioritise a busy workload and have sound ICT skills.

Main duties of the job
- Support the ward manager and provide effective clinical leadership to the immediate clinical team members
- To assist the deputy ward managers in developing a supportive and cohesive team whose primary purpose is to maintain and deliver excellent standards of care
- To undertake the role of Nurse in Charge as and when required
- The focus of the role is the day-to-day delivery, of excellence in care provision
- Ensure the observation and supervision of patients and report any identified concerns to the ward manager or Registered Manager in a timely manner
- Ensure that patients are supported to maintain independence, social contact, privacy and dignity
- Identify realistic goals of care and outcomes with patients, ensuring that the patient fully participates and agrees with plans of care
- Record explicit care plans which reflect the agreed goals and outcomes, which act as effective tools for all the multi-disciplinary team
- Measure the effectiveness of the care plan in achieving the identified goals and to revise and update care plans accordingly
- Participate in the induction, mentorship, teaching and support of new staff and students on placement, acting as a positive role model to all staff
- Support, guide and assist Health Care Assistants in carrying out their role in caring for patients
