CNS/CPS - Hospice Outreach Service
As part of our model of care to ensure we have the right people, providing the right care in the right place and at the right time, an opportunity has arisen for a Clinical Nurse Specialist (CNS) or Clinical Paramedic Specialist (CPS) to work as part of our multi-disciplinary team within the Hospice Outreach Service in the community covering West Kent and East Sussex.
The Hospice Outreach Team is based in the Pembury site of Hospice in the Weald. The team provides the single point of access for all referrals into all services for Hospice in the Weald.
The Hospice Outreach Service provides specialist palliative advice and support to patients and their families. You will be part of a rota of clinicians covering our 7 day a week service offering home-based assessments in addition to telephone support.
Our team care for patients who have a life limiting illness and our focus is on individualised care, focusing on improving quality of life and achieving patient centred goals. We care for people with malignant and non-malignant long term conditions including heart failure, respiratory disease, frailty and neurological conditions. Our team works collaboratively with our external partners to provide seamless care within the patients journey; this enhances collaborative working and provides opportunities for shared learning.
Main duties of the job
To succeed in this role
We are looking for a motivated clinician with the ability to work as an autonomous practitioner whilst being part of a responsive and resilient team. You will possess advanced communication skills and be a supportive team player.
You will have proven community or in-patient palliative care experience at Band 6 or 7 or have equivalent level of experience of working in the following related areas: Paramedic, General Medicine, Neurology, Emergency Medicine, Frailty, Respiratory Care, Cardiology or Oncology. You will have advanced assessment skills and non-medical prescribingorbe willing to undertake as part of professional development.

The Company
Hospice in the Weald was founded in 1980 as a small home-care service in West Kent, born out of local community effort to support people with life-limiting illnesses.
Over the decades it has grown from those humble beginnings into a vital palliative care charity serving West Kent and northern East Sussex, expanding its services to include an in-patient ward, day services, outreach and counselling support, all free of charge, as well as innovative new provisions such as childrens care and the Cottage Hospice at Five Ashes.
Today, more than 40 years on, Hospice in the Weald continues to provide compassionate, personalised care to patients and their loved ones, celebrating life and offering dignity at the end of life for thousands in the local community.
