PSALT Community Substance Use Nurse

Job summary

Role Summary

We are looking for an enthusiastic person centred registered nurse to delivery community drug services within the Swansea region. Kaleidoscope PSALT is a GP with Specialist Interest led service for individuals resident in Swansea aged 18 years +. Opiate Substitution medication is dispensed via a community pharmacy model. Excellent communication between primary care, secondary care and pharmacy colleagues will be required. The substance use nurse will assess, review and co-ordinate prescription changes with the support of a multidisciplinary team. They will deliver harm reduction interventions including Blood Borne Virus screening, vaccinations, overdose prevention and sexual health referrals.

Main duties of the job

Key Responsibilities

  • Undertake comprehensive clinical assessments Facilitate outreach and community engagement to assess, commence and monitor opiate substitute therapy (OST)
  • Provide advice, information, and guidance on the management of substance use to service users engaged with services.
  • Deliver subcutaneous injection of buprenorphine (Buvidal).
  • Follow Controlled Drug Policies and Procedures.
  • Agree treatment plans with service users, and review them regularly
  • Undertake drug screening (urine, fingerprint and oral).
  • Ensure case notes and other statutory records are up to date and kept in accordance with Kaleidoscope policies and procedures / regional information sharing protocols
  • Deliver clinical interventions, including HCV testing, HBV immunisation and naloxone training/administration where appropriate and in partnership with Local Health Board and Public Health Wales colleagues. Liaise with community pharmacies to ensure prescriptions are in place and monitored appropriately.
  • Provide advice, support and training to GPs, pharmacists, and other professionals where appropriate.
  • Participate in clinical audits and service reviews
  • Lead with referrals / liaison with Community Mental Health Teams (CMHTs) and other community substance use treatment providers on entering and leaving Kaleidoscope services. Actively encourage service user involvement and feedback and ensure views are considered in informing the development of the service.
  • Liaise with primary care and sexual health services.

  • Experience of developing and maintaining successful partnerships with GPs, treatment agencies and other healthcare providers.

  • RGN/RMN with valid NMC Pin.
  • Relevant specialist training / experience in alcohol screening / brief advice / drug treatment services

  • Independent / supplementary nurse prescribing qualification.
  • Phlebotomy qualification / registration
