Job summary

You will be responsible for the processing of referrals to our Hospice services. You will be the first point of contact and will carry out the initial clinical triage with patients and other professionals. You will disseminate relevant information to the multi-disciplinary team to support timely and appropriate care.

The role requires organisational skills, flexibility and the ability to work in an unpredictable environment. Experience managing patients with palliative care needs and a palliative care qualification is advantageous, but we recognise that all experience is transferable and offer the opportunity to develop your knowledge and skills.

Main duties of the job

To co-ordinate all referrals received for all hospice services ensuring patients receive timely and appropriate care.

About us

This role is based at Springhill Hospice, Rochdale.

Springhill Hospice provides the following palliative care services to the local adult population who are suffering from the effects of life-limiting and terminal illnesses:

A 16-bed Inpatient unit providing 24-hour medical and nursing care.

Hospice at Home Service - supports people who want to end their days in their own home by providing the support and symptom management they need to be able to do so.

A Day Therapies Centre for up to 16 patients, 4-days-a-week, offering complementary therapies (e.g. reflexology, aromatherapy and massage); creative and beauty therapies, social activities and clinical support.

Community Service - provides specialist palliative care and support to patients and families in their own homes

A 24-hour Palliative Care Advice Line for patients, families, carers and health professionals.

Bereavement Support for relatives following loss.

Counselling Services for patients, carers and relatives, provided by a qualified therapist.

An Education Unit - an in-house training facility for hospice staff and local health and social care professionals, with the main focus of training being on the Springhill Palliative Care Education Passport' (SPCEP) which ensures that core palliative care skills are taught to all grades of staff.

Job responsibilities

Professional

3. In liaison with the hospice multi-disciplinary team, facilitate efficient and timely access to all services including admission to the inpatient unit.

4.Provide specialist support and advice to patients, families, hospice staff, volunteers and other healthcare professionals.

5.Where immediate access to services is not possible, explore other sources of support for the patient in liaison with community health and social care teams.

6.Promote and maintain effective communication with external agencies i.e. Macmillan, district nursing, Specialist nurses, Acute trust.

7.Work closely with the wider multi-disciplinary team to provide expertise, knowledge and support to patients, families and carers who require support in the community.

8.Promote and take an active part in the 24-hour advice line service, providing advice line staff with relevant information relating to patients and ensuring information relating to telephone calls is appropriatelycommunicatedto other healthcare professionals involved in the patientscare.

9.Ensure close communication, liaison and joint working with members of the clinical team within the Hospice.

10.Maintain comprehensive documentation relating to patient referrals by use of the iCare system.

11.Ensure that patient records are maintained in accordance with both statutory and Hospice requirements.

12.Inline with quality monitoring andgovernance, to continually evaluate policy, procedure and documentation relating to the role, through relevant sub-committees and governance groups.

Education

1.Ensure attendance at all mandatory training and development sessions required by Springhill Hospice.

2.Contribute to the training and development of other staff within the hospice and to provide supervision and support for all learner grades of staff including external students on placement.

3.Take responsibility for identifying own personal and professional development requirements and communicating these to the line manager.

4. Maintain an active role in self-development and continuing education by attending planned educational sessions and development training.

Learning and Development

1.Maintain own professional development in order to support service delivery across the organisation.

2. Actively participate in the Hospice appraisal processes to identify own and others learning needs and take necessary steps in order to reach personal, professional and organisational objectives.

3. Practice in accordance with NMC Code of Conduct and ensure that professional registration is maintained.

4. Work in accordance with Hospice policies to ensure that confidentiality is maintained at all times by self and others.

5.Keep up to date with current palliative care practice and professional development and conduct for self and others.
