Job summary

We are looking to appoint a dynamic experienced and highly motivated nurse to join our Motor Neurone Disease Community Nurse Specialist Palliative Care Team (LOROS). We provide care to individuals throughout Leicester, Leicestershire and Rutland. The team also participate in training programmes for both professionals and non-professionals. There will be opportunities for self-development and involvement in service development and clinical audit.

This post holder will act as a key worker and support patients with Motor Neurone Disease (MND) and their families from diagnosis to end of life. You will work alongside other members of the multi-disciplinary team both at LOROS and across the wider health and social care community, including the neurology department at UHL and social services.

Main duties of the job

You will need to have well-developed interpersonal skills, experience and be passionate for caring for patients with MND and/or other neurological conditions. Community experience and experience of supporting patients with palliative and end of life care needs would also be beneficial.

This is an excellent opportunity to gain experience in a highly specialist palliative care team, supporting patients living with MND and making a real difference during critical time in their lives.

Due to the nature of the role and visiting patients in remote areas it is essential that you hold a current driving licence and have access to a car/transfer for work purposes.

Job description

Job responsibilities

  • Undertake home visits and complete holistic specialist needs assessments.
  • Provide specialist intervention to support patients and carers.
  • Act as a resource for the primary health care team, other specialist team's statutory organisations and voluntary organisations on issues relating to the care of patients with MND.
  • Provide a telephone advice service to patients, carers and professionals.
  • In collaboration with the multi-professional team, plan the provision of care based on a holistic needs assessment.
  • Manage nurse led MDT meetings and undertake nurse led MND clinics.
  • Liaise with other staff and departments at the Hospice and refer to other agencies that can provide additional forms of support for example MNDA.
  • In conjunction with other members of the Multi-disciplinary Team (MDT), monitor the progress of all the MND patients.
  • Provide information to patients and carers to enable them to make informed choices regarding care.
  • Comply with the NMC code of practice and adhere to all policies and procedures of the Leicestershire & Rutland Hospice.
  • Comply with the Health and Social Care Act 2008/Care Quality Commission.
  • According to LOROS Hospice Policy and where the Nurse Specialist has undergone training, prescribe medication for patients.
