We are seeking an experienced and motivated Lead Practice Nurse for 30-37.5 hours per week to join our friendly NHS practice and play a key role in shaping and supporting our nursing team at Manor Park Surgery.

Main duties of the job
The postholder will be responsible for leading the delivery of nursing services at the practice, working as part of the practice multidisciplinary clinical team and delivering nursing care within their scope of practice to the patient population. This will include, but is not limited to assessing, examining and managing patients with chronic disease, wound care, cervical cytology, family planning, travel health and child & adult immunisation. The postholder will also have responsibility for leading and managing the nursing team in collaboration with the Senior GP Partner. The Lead Nurse will provide support, training and mentorship for the clinical team, ensuring the appropriate skill mix to meet the nursing needs of the practice population. They will be involved in the strategic development of nursing services at the practice, developing and reviewing clinical policy and procedure, participating in audit, case finding and clinical meetings, prevention, as well as supporting the management team in the reviewing of clinical policy and procedure.

Manor Park Surgery is a GP surgery based in Bramley, Leeds. We are a four GP partner training practice serving a population of circa 14,000 patients, and we use SystmOne. We offer training for medical students, nursing students and GP registrars. We are a forward thinking, multi-disciplinary team and we work supportively with our Care Navigation and Administration teams. We have a purpose-built, two storey building with on-site parking facilities and we are rated good by the CQC.

To support and influence the role of our nursing team within any clinical service redesign plans both within the Practice and also within the wider Pudsey Primary Care Network
Provide leadership in areas related to infection prevention and control and emergency equipment, including being the named Infection Prevention and Control lead for the Practice, ensuring compliance and oversees audit as necessary

Clinical Duties
The content of clinical duties will depend on the specific skills, knowledge and experience of the Lead Nurse. This can include
Case management of long term medical conditions in a practice setting, providing subject matter expert advice to patients, evaluating individual specialised treatment plans and referring patients to secondary or specialist care as required
Identifying, managing and supporting patients at risk of developing long-term conditions, preventing adverse effects on the patients health
Practice Nursing
Leading on quality and performance of services that are primarily provided and monitored by the Nursing Team e.g. immunisations, cervical screening, flu, among others, with emphasis on CQC related improvement
Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, SIGN, BTS, NICE
Maintain accurate clinical records, ensuring effective use of Read codes
Maintain chronic disease registers
Support the team in dealing with clinical emergencies
Recognise, assess, and refer patients presenting with mental health needs
Implement vaccination programmes for adults and children
