Job summary
We are looking to recruit a friendly, motivated Practice Nurse to join our supportive primary care team, following the well-deserved retirement of a long-standing colleague.
The Mote Medical Practice joined forces with Staplehurst Health Centre last year, so this is an exciting time of growth and development, with opportunities to help shape and refine ways of working as the team continues to settle and evolve.
The role involves delivering high-quality, patient-centred care across chronic disease management, health promotion, and routine nursing clinics, and would suit someone who is adaptable, collaborative, and keen to be part of a practice that is actively building its future.
Main duties of the job
Provide high-quality, patient-centred nursing care within the general practice setting
Manage chronic diseases (e.g. diabetes, asthma, COPD, hypertension) in line with clinical guidelines
Deliver health promotion and disease prevention, including lifestyle advice and screening
Undertake immunisations and vaccinations across all age groups
Run treatment and review clinics, such as cervical screening, wound care, and contraception support
Monitor patients long-term conditions, including observations, reviews, and care planning
Work collaboratively within the multidisciplinary primary care team to ensure coordinated care
Maintain accurate clinical records and contribute to audit, QOF, and quality improvement
Support patient education and self-management, empowering patients to manage their health

Job responsibilities
The
following are the core responsibilities of the Practice Nurse. There may be, on
occasion, a requirement to carry out other tasks. This will be dependent upon
factors such as workload and staffing levels:
a.
To support the Infection
Prevention Control (IPC) lead and to work to the IPC Policy supporting the implementation of
audits, training, support and evidence of compliance to the practice manager
b.
To support the production of
the IPC annual statement in conjunction with the practice manager
c.
To
assess the needs of patients ensuring the provision of healthcare is
appropriate, incorporating evidence-based practice
d.
To
develop, implement and embed health promotion and wellbeing programmes
e.
To
implement and evaluate individual treatment plans for chronic disease patients
that promote health and wellbeing
f.
To
identify, manage and support patients at risk of developing long-term
conditions, preventing adverse effects to patients health
g.
As
required, to provide routine nursing care to patients in accordance with
clinical based evidence, NICE and the NSF
h.
To
co-ordinate the provision of travel medicine services
i.
To
request pathology services and process pathology results as required
j.
To
oversee the provision of chronic disease clinics, delivering patient care as
necessary
k.
To
maintain accurate clinical records in conjunction with extant legislation
l.
To
ensure read codes are used effectively by all members of the nursing team
m.
To
ensure the appropriate maintenance and use of chronic disease registers
n.
To
prioritise health issues and intervene appropriately
o.
To
support the team in dealing with clinical emergencies
p.
To
recognise, assess and refer patients presenting with mental health needs
q.
To
implement vaccination programmes for adults and children

To
support patients in the use of their prescribed medicines or over-the-counter
