Job summary

The post holder will be accountable for ensuring the highest standard of clinical care is provided within the relevant setting. To work as an autonomous nurse prescriber, responsible for the delivery of nursing services, working as part of the practices multidisciplinary team, delivering care within their scope of practice, to the entitled patient population.

The post holder emphasis will be on patient safety, patient experience, patient engagement, and clinical outcomes. The postholder will have clinical responsibility for supporting the partners in activities relating to the delivery of high-quality services, including involvement in maintaining activity levels, managing service pressures and maintaining the highest standards of care.

The post holder will be responsible for a number of clinical areas such as minor illness clinics, health promotion, Diabetes care, chronic disease management, health prevention, as well as supporting the partners and clinical teams in reviewing clinical policy, protocols and procedures to deliver high quality sustainable care for patients.

The post holder will have developed skills across a wider range of subject matter, including diagnostic and therapeutic interventions.

Main duties of the job

Conduct comprehensive assessments of patients.Prescribe, review medications for therapeutic effectiveness.Work with doctors, nurse practitioners and pharmacists. Job description

Job responsibilities

Conduct comprehensive assessments of patients to determine their healthcare needs and develop appropriate treatment plans.

Prescribe and review medications for therapeutic effectiveness, ensuring they align with evidence-based practices and national guidelines.

Work closely with doctors, nurse practitioners and pharmacists to ensure coordinated and safe patient care.

Manage both acute and chronic conditions, integrating drug and non-drug treatment methods into care plans.

Conduct health screening and preventative care measures for the enrolled patient population

Maintain accurate and up-to-date patient records, documenting assessments, treatment plans and outcomes.

Provide health education and advice to patients regarding their conditions, treatment options and medication management

Complete management of episodes of care and high level complex clinical decision making.

Provide clinical support to Practice Nurses, HCA and other colleagues as required.

Provide clinical support for colleagues for the following: wound care advice, INR dosing, BP monitoring and titration, contraception advice (list is not exhaustive).

Work within broader scope of the practice, able to lead in situations that are complex and unpredictable and offer supervision to others.

Work with the clinical team to provide appropriate staffing on both a daily and longer-term basis.

Support the Practice Manager to ensure Systm1 scheduled rota appointments align with QOF requirements.

Attend monthly clinical meetings, provide clinical service updates as required.

Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice to patients.

Evaluate and implement individual specialist treatment plans for chronic disease patients.

Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients' health.

Run Minor illness clinics.

Provide wound care.

Provide specialist clinics delivering patient care as necessary, referring patients to secondary/specialist care as required.

To complete medication authorisations for patients.

Prescription signing.

Provide general clinical advice for queries for all members of the team.

Take calls from outside members of the multi-disciplinary teams, work with external agencies.

Participate in the review of significant and near-miss events applying a structured approach i.e. root cause analysis.

Clinical Quality and Patient Experience

Address the concerns of patients and services users and respond to their suggestions for local quality improvements in services.

Work with the Practice Manager and clinical teams to ensure the effective resolution of complaints and any subsequent learning, undertaking risk management and reviewing patient experience as necessary.

Act as a positive clinical role model. Lead by example to motivate others, ensuring the highest standard of care.

To work with the clinical team to ensure the standards of cleanliness and hygiene in their areas comply with policies and procedures.

Support the development of protocols, process and best practice to ensure the highest standard of care.

Maintain accurate clinical records in conjunction with extant legislation.

Support on CQC compliance for clinical best practice, ensuring process and policies align to CQC compliance standards.

Support the Practice Manager with developing and maintaining robust CQC standards, reviewing current processes and policies.

Work with the Practice Manager on Quality and compliance standards, issues and risk.

Safeguarding

To support the delivery of Safeguarding Adults and Children Governance framework and processes within the organisation. Encourage the clinical team to promptly escalate highly complex safeguarding cases to the Practice Manager.
