Job summary
We are looking for an experienced HCA/Care Coordinator to work
15-20 hrs per week over 2-3 days to support our friendly practice nursing team in
the delivery of our nursing services. The successful candidate will be working
as part of the practice multidisciplinary team, delivering care within their
scope of practice to our patient population. The successful candidate will
deliver and assist clinical staff in the provision of treatment, preventative
care, health promotion and patient education.
Main duties of the job
Working under the direct supervision of the GPs and
existing HCA and strictly in accordance with specific practice guidelines and
protocols, the healthcare assistant will assist the practice clinical team in
the provision and delivery of prescribed programmes of patient care. Directing
patients to locally available support and services whilst using clinical
systems on a daily basis, with confidence to learn advanced techniques and keep
care records up to date. Supporting the coordination of all key
activity, including access to services, advice and information, as well as
ensuring that health and care planning is timely, efficient, and patient centered.

Control Policy.

re-stocking and stock rotation
- Infection Prevention
Control / Undertake housekeeping duties, including general tidiness and
cleanliness of treatment and GP rooms.
