Job summary

We are looking for an experienced HCA/Care Coordinator to work 15-20 hrs per week over 2-3 days to support our friendly practice nursing team in the delivery of our nursing services. The successful candidate will be working as part of the practice multidisciplinary team, delivering care within their scope of practice to our patient population. The successful candidate will deliver and assist clinical staff in the provision of treatment, preventative care, health promotion and patient education.

Main duties of the job

Working under the direct supervision of the GPs and existing HCA and strictly in accordance with specific practice guidelines and protocols, the healthcare assistant will assist the practice clinical team in the provision and delivery of prescribed programmes of patient care. Directing patients to locally available support and services whilst using clinical systems on a daily basis, with confidence to learn advanced techniques and keep care records up to date. Supporting the coordination of all key activity, including access to services, advice and information, as well as ensuring that health and care planning is timely, efficient, and patient centered.

Job responsibilities

  • Working with GPs and other professionals within the practice to identify and manage a caseload of patients, particularly vulnerable patient groups with long-term conditions that may require extra support.
  • Directing patients to locally available support and services.
  • Supporting the coordination of all key activity, including access to services, advice and information, as well as ensuring that health and care planning is timely, efficient, and patient-centred.
  • Assisting the Nursing Team with patient wound assessment and care
  • Phlebotomy
  • Vaccinations and injections where qualified eg Influenza & B12
  • Chaperoning duties
  • Promoting health education.
  • Maintain and clean equipment used by the nurses and GPs according to the practice Infection Control Policy.
  • Surgical equipment re-stocking and stock rotation
  • Infection Prevention Control / Undertake housekeeping duties, including general tidiness and cleanliness of treatment and GP rooms.

  • GCSE grade A to C in English and Maths
  • Strong IT skills
