Advanced Nurse Practitioner

Job summary

We are looking for a highly motivated and autonomous Advanced Nurse Practitioner to work 8 sessions per week (across 4 days). You will be supported by an established team of GPs, nurses, HCAs, and administrative staff, and play a vital role in delivering safe, efficient, and high-quality care to our patients.

Main duties of the job

Key Responsibilities

  • Provide comprehensive clinical assessment, diagnosis, and management of patients presenting with both acute and chronic conditions.
  • Undertake telephone and face-to-face consultations.
  • Prescribe medications and request/interpret investigations as appropriate.
  • Support the management of long-term health conditions.
  • Contribute to QOF, enhanced services, and practice development initiatives.
  • Work collaboratively with the wider clinical team and PCN to deliver patient-centred care.
  • Assist with oversight and development of our current nursing team.

  • Autonomous Patient Care:

Manages the complete clinical care of patients, from initial assessment and diagnosis to treatment and evaluation.

  • Clinical Assessment:

Conducts in-depth patient histories, physical examinations, and interprets diagnostic tests to identify and manage both acute and chronic illnesses.

  • Diagnosis and Treatment:

Diagnoses a range of conditions, develops specialised treatment plans, and prescribes appropriate medications.

  • Health Promotion & Prevention:

Delivers health promotion services and supports patients in managing chronic conditions and reducing risks for future health problems.

  • Multidisciplinary Teamwork:

Collaborates closely with GPs and other healthcare professionals within the general practice team to ensure holistic care.

  • Record Keeping:

Maintains accurate and detailed clinical records in compliance with legislation and uses appropriate coding systems.

  • Advanced Clinical Expertise:

Requires master's-level education and proven competency in advanced clinical decision-making, assessment, diagnosis, and treatment.

  • Prescribing Authority:

An extended or supplementary prescriber, able to prescribe independently according to clinical guidance.

  • Communication:

Possesses highly developed communication, negotiation, and interpersonal skills to discuss complex issues with patients and colleagues.

  • Holistic Approach:

Applies a holistic approach to healthcare, integrating nursing and medical models to provide comprehensive care.

  • Research and Evidence-Based Practice:

Utilises an evidence-based approach to care and service development, applying research findings to patient management.
