Practice Nurse

Job summary

We are looking for a Practice Nurse to work in our busy general practice (GP) surgery, providing a wide range of clinical services and health education to patients across all age groups. The successful candidate will play a key role in supporting our GPs, managing long-term conditions, promoting health and wellbeing, and delivering preventative care.

Main duties of the job

Key Responsibilities

  • Administering vaccinations and injections (e.g., flu, travel, childhood immunisations)

  • Managing chronic diseases such as diabetes, asthma, and hypertension

  • Conducting cervical smear tests and women's health clinics

  • Providing wound care and minor injury treatment

  • Performing health screenings

  • Assisting with family planning and sexual health services

  • Educating patients on lifestyle, diet, smoking cessation, and disease prevention

  • Supporting patients with mental health or social care needs

  • Collaborating with GPs and other healthcare professionals for patient care plans

Skills and Qualities
  • Strong clinical and decision-making skills

  • Excellent communication and interpersonal abilities

  • Organised and able to work independently

  • Empathetic and patient-centred

  • Knowledge of primary care protocols and long-term condition management

Qualifications

  • Registered Nurse (RN) with the NMC (Nursing and Midwifery Council)

  • Additional training in areas such as immunisation, chronic disease management, or cytology will be provided where required.

A friendly team with 4 GP Partners, regular GP Trainees, a nursing team comprised of Nurse Practitioners, Practice Nurse & Health Care Support Workers. Our admin team is made up of Receptionists, Administrators, Reception manager, Finance manager and Business manager. Reports to: Nurse Lead / Business Manager

Location:Leek Health Centre GP Surgery

Salary: Dependent on experience and qualifications

Hours:Part-time

Job Summary:

The Practice Nurse plays a key role in delivering primary healthcare services within a general practice setting. They are responsible for assessing, screening, treating, and educating patients across a range of health conditions, supporting both acute and chronic disease management. Practice Nurses work collaboratively with GPs, healthcare assistants, and other multidisciplinary team members to provide high-quality patient care.

Key Responsibilities:

Clinical Duties

  • Provide general and specific nursing care, including:

    • Immunisations and vaccinations (childhood, travel, flu, etc.)

    • Cervical smears

    • Wound care and dressings

    • Removal of sutures/clips

  • Manage and support patients with chronic conditions:

    • Diabetes

    • Asthma

    • COPD

    • Hypertension

  • Perform health checks and screenings

  • Provide contraception advice (as trained)

  • Carry out ECGs and phlebotomy (if required)

  • Administer and manage medications and injections (e.g. B12, contraceptives)

Health Promotion and Patient Education
  • Advise patients on lifestyle changes including:

    • Smoking cessation

    • Diet and exercise

    • Alcohol use

  • Educate patients on self-care and disease prevention

Administrative and Professional Duties
  • Maintain accurate patient records using EMIS

  • Participate in audits, clinical meetings, and quality improvement initiatives

  • Ensure compliance with infection control and health & safety protocols

  • Support the practice in meeting QOF, CQC, and other regulatory standards

Qualifications and Requirements:
  • Registered Nurse (RN/RGN) with current NMC registration

  • Post-registration qualifications in areas such as asthma, diabetes, or cervical cytology (desirable but training can be provided)

  • Previous experience in a primary care or community setting (preferred)

  • Excellent communication and interpersonal skills

  • Ability to work independently and as part of a team

  • Commitment to continuous professional development

Desirable Attributes:

  • Experience using clinical IT systems (e.g., EMIS)

  • Flexible and adaptable approach to changing demands
