Job summary

Can you juggle tasks with precision, communicate well and remain steady in fast-paced situations? If your answer is yes you could be the perfect fit for our team as we seek to recruit 2 part-time Care Navigators.

Our team is a friendly, forward thinking GP Practice where patient care and team wellbeing come first.

These new roles have been designed to support our clinical team in effectively and safely navigating patients through a variety of complex care pathways.

Each post holder will cover Monday to Friday for 27.5 hours per week. The working pattern of each postholder will be based on a 2 week rota (i.e. Week 1 M-F 0800-1330 and Week 2 M-F 1300-1830).

These new roles are critical to patient safety. Therefore both Care Navigators will be required to provide cross cover for each other. This may include a temporary adjustment to your working pattern to cover core hours of 0900-1700 Monday to Friday. Any additional hours worked will be compensated accordingly.

Main duties of the job

Patient Navigation: Support the triaging clinician/s in navigating patients accessing the services at Sharnbrook Surgery, to ensure efficient access of healthcare services. This includes prompt response to triage outcomes such as gathering key information and directing and progressing patients through the appropriate pathway and/or to the appropriate service based on their needs.

Support Total Triage: Be the primary administrator assisting in the Total Triage model by assessing requests and ensuring they are handled safely and promptly.

General Administrative Duties: Manage appointments, maintain accurate patient records and handle incoming calls and correspondence. This includes processing administrative requests such as insurance forms, DVLA forms and Subject Access Requests.

Communication: Provide clear and empathetic communication with patients and colleagues, addressing queries and concerns while maintaining confidentiality and professionalism. This includes written, telephone and face to face interactions.

Collaboration: Work closely with clinical and non-clinical colleagues to ensure a smooth patient journey and support the overall operations of Sharnbrook Surgery. This includes internal and external colleagues.

Primary Responsibilities

Co-ordinate and monitor Total Triage inbound screen ensuring all triage requests are directed to the appropriate clinician promptly upon receipt.

Co-ordinate and monitor Total Triage outcomes ensuring all actions and appointments are processed efficiently.

Maintaining and monitoring the practice appointment system i.e. make / cancel / rearrange appointments via the clinical system

Enter data of new and temporary registrations and relevant patient information as required ensuring accuracy.

Scan patient medical records, results and letters as required.

Answer incoming phone calls, transferring calls or dealing with the callers request appropriately.

Signpost and support patients to the correct services as advised and/or as per pre-determined pathways.

Ensure patient information is always up to date and accurate.

Greet patients and visitors arriving at the reception desk, ensuring they have checked in. As appropriate ensure support is required in these tasks.

Observe patients and visitors in the waiting room and alert an appropriate member of staff to any concerns or issues. This includes any wellbeing and security concerns.

Ensure the waiting and reception areas are kept tidy and that information displayed is up to date.

Ensure effective communications, regularly updating the Team Leader of any issues.

Initiating contact with and responding to, requests from patients, team members and external agencies.

Always ensure confidentiality and professionalism with patients, visitors and colleagues.

Assist in maintaining the safety of staff, patients and visitors.

Always maintain a clean, tidy and effective working area, ensuring compliance with the Surgerys clear desk policy at the end of each working day. This includes wiping down workstation to maintain infection, prevention and control standards.

Carry out any other duties as reasonably requested by colleagues.

Direct requests for information i.e. SAR, insurance / solicitors letters and DVLA forms to the GPAs.

Manage all queries as necessary in an efficient and timely manner.

Support colleagues throughout the surgery with general tasks as requested and appropriate

Secondary Responsibilities

Partake in audit as directed.

Provide cross cover during planned and unplanned absences.

Arrange and take minutes at meetings as required and appropriate.

Raise orders for surgery consumables such as toners, hand towels etc. as instructed and required.

Complete opening and closing procedures in accordance with the duty rota.
