Lea Wharf Medical in Hertford are looking for a Prescribing Pharmacist to support the work of our clinical team. We are developing a multidisciplinary team to provide appropriate care for our growing population.
We are a Patient Facing Clinical Services and Care Team based in an innovative Primary Care Practice looking to enhance the skill mix of permanent personnel within our team. Our team is consist of over 40 people and include GPs, Nurses, Health Care Assistants and an Emergency Care Practitioner, along with non-clinical team of receptionist and administrators.
Main duties of the job
This will be a fixed term contract for 12 months.
Permanent Job Purpose To improve patients health outcomes and the efficiency of the Primary Care Team. To provide direct, accessible and timely medicinal expertise. To pro-actively transfer workload relating to medication optimisation issued from other clinical staff to improve patient care, safety and free up clinical staff to spend more time on clinical care. To support long term condition and minor illness services. To support patients in residential homes. To integrate fully in to the Primary Healthcare team and support the design and delivery of safe process in prescription management. .Essential for this role:
- Previous experience working in a clinical environment
- Excellent communication skills.
- The confidence to discuss all medication related issues with stakeholders
- A commitment to lead innovation
- Qualified pharmacist with valid GPhC registration
- Two Professional Satisfactory References
- Satisfactory Registration with the DBS Update service or a satisfactory DBS dated within the past 12 months
- A complete CV covering the whole period from registration to present day.

They will manage undifferentiated presentations, recognise clinical risk, and escalate to the GP or other services when required. The role includes requesting and interpreting investigations, prescribing within scope of practice (where qualified), and supporting patients with health advice and self-care guidance.

- Applicants must be HCPC-registered Paramedics with experience in clinical assessment and autonomous decision-making. Experience in primary care is desirable, as is an independent prescribing qualification or willingness to work towards one. The successful candidate will be professional, adaptable, patient-focused, and able to work both independently and as part of a team.
Desirable
- The Paramedic Practitioner will work closely with GPs and the wider multidisciplinary team, contribute to clinical discussions, and maintain accurate clinical records in line with clinical governance requirements.
- Experience working in primary care or general practice.
- Experience supporting training or supervision of junior pharmacy staff.
- Familiarity with prescribing data systems and audit tools.
- Independent Prescriber Status recognised by the GPhC.
