Job summary
To be responsible for implementing an effective medicine management system within the practice, including cost-effective prescribing, working as an independent prescriber, providing advice and support for both patients and colleagues. In addition, the post-holder will also support the multi-disciplinary team, in line with the strategic objectives of the practice.

Key Responsibilities
Patient-Facing Clinical Medication Review Undertake clinical medication reviews with patients, work within your scope of practice to implement any necessary changes reviews could be cohort based within the pharmacists competence. Lead and coordinate medicines optimisation services and clinical pharmacy development to improve quality of care, personalised care and patient experience.
Undertake clinical audits in, feedback the results and implement changes in conjunction with the relevant practice team. Identify cohorts of patients at high risk of harm from medicines through computer searches. This might include risks that are patient related, medicine related. Put in place changes to reduce the prescribing of these medicines to high-risk patient groups. Identify, plan, organise, and co-ordinate the implementation of medicines optimisation initiatives, objectives and complex changes at GP practice to bring about quality improvements in prescribing behaviour and better outcomes for patients including reducing patient harm and improving medication safety.
Provide specialised advice and support in risk management and clinical governance issues and ensure compliance with medicines legislation, patient safety advisory notices.
Review progress yearly and develop clear plans to achieve results set by others.
Attend and contribute to practice, Clinical Meetings when relevant.

Acting as the first point of contact for medical advice, treatment, managing both acute and long-term conditions. Services include health advice, vaccinations, screenings, prescriptions and referrals to specialist.
Patients are able to book appointments online, by phone or in person. All are processed through our triage system.
Along with the Time to learn time allocation each month, we provide clinical meetings monthly to keep staff up to date with implemented changes along with presentations from various consultants Our team work extremely well as one to ensure we provide the best care for our patient population.
