PCN Paramedic/Paramedic Practitioner

Job summary

The post holder will be an experienced clinician, who, acting within their professional boundaries will provide clinical assessment and management of patients.

They will support the delivery of high-quality care and will undertake service development and improvement of the local unscheduled care system, working within the team, across practices and with partners. Committed to the development of innovative practice and expanding evidence-based care which will contribute to the enhancement of the paramedic practitioner role.

You will be expected to provide and maintain a high standard of care for patients as well as providing support to the GPs and other members of the primary health care team. Supported by a skill mixed team, they will work within the practice, as an autonomous, accountable, Paramedic Practitioner, in the provision of a holistic approach for patients including assessment, diagnosis and treatment, to deliver quality patient services.

Advanced paramedic practitioners work autonomously within the practice, using their enhanced clinical assessment and treatment skills, to provide first point of contact for patients presenting with undifferentiated, undiagnosed problems relating to minor illness or injury. They are health professionals who practice at an advanced level having the capability to make sound judgements in the absence of full information and to manage varying degrees of risk when there are complex, competing or ambiguous information or uncertainty.

Main duties of the job

They will assess and triage patients, including same day triage, and as appropriate provide definitive treatment or make necessary referrals to other members of the primary care team.They will advise patients on general healthcare and promote self-management where appropriate, including signposting patients to other community or voluntary services.

They will be able toperform specialist health checks and reviews,perform and interpret ECGs;perform investigatory procedures as required, and;undertake the collection of pathological specimens including intravenous blood samples, swabs etc.perform investigatory procedures needed by patients and those requested by the GPs

They will support the delivery of anticipatory care plans and lead certain community services

They will provide an alternative model to urgent and same day home visits for the network and undertake clinical audits

They will communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required

They will maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice

Rochester Road surgery is a friendly practice and part of Gravesend Central PCN. It is a small sized practice located over two sites. The clinical team is known for excellen patient care and it is a high acheiveing practice. There are opportunities for staff development and team working.

The following sets out the key role responsibilities for paramedics:

a. They will assess and triage patients, including same day triage, and as appropriate provide definitive treatment or make necessary referrals to other members of the primary care team.

g. They will communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required

h. They will maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice

i. if the post holder is a non-medical prescriber then to be able to prescribe/issue medications as appropriate following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways

j. Enhance own performance through continuous professional development, imparting own knowledge and behaviours to meet the needs of the service.

Act within acceptable parameters as an employee/ Manager for the organisation, having regard to the applicable Code of Conduct for your role and ensuring own practice is in accordance with organisational policies.

Maintain own CPD and contribute to own personal development by participating in annual appraisal with line manager, developing a PDP, and actively participating in agreed learning activities and evaluating effectiveness of learning in relation to role.

Regularly review own practices and makes changes in accordance with current and/or best practice, makes suggestions for improved practice and identifies where other activities affect own practice.

To achieve and demonstrate agreed standards of personal and professional development within agreed timescales.

To contribute positively to the effectiveness and efficiency of the service in which he/she works.

For paramedic practitioners:

a. has a masters degree level in the relevant area of expertise;

b. is working at a masters level aware or equivalent that encompasses thefour pillars of clinical practice, leadership and management, educationand research, with demonstration of core capabilities and area specificclinical competencies
