Job responsibilities

To provide a high-quality, cost-effective service to the patients of Stowhealth diagnosing and prescribing treatments and medication while working within defined procedures and protocols as laid down by the GPs.

To work as part of a clinical team, with GPs, Advanced Nurse Practitioners, Physician Associates, Practice Nurses and Healthcare Assistants.

To be responsible for seeing patients and visiting patients in their place of residence, who present with a wide range of minor illness, urgent and same day requests for treatment and chronic disease management; providing care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.

The post holder will need to demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills. There will be the need to prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner.

Key Responsibilities

Provide assessment, diagnosis and treatment at first point of contact by attending to patients according to their needs either in the practice sites or patients homes. Act as an autonomous clinician, with the support and guidance from the GPs

Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).

Diagnose and manage both acute and chronic conditions, integrating both drug and non-drug based treatment methods into a management plan.

Accurately triage and prioritise patients, including in emergency situations, demonstrating the use of a variety of techniques to elicit the history of an event/illness, including past medical and drug history.

Recognise, assess and refer patients presenting with mental health needs

Telephone triage as appropriate for clinics and home visits and provide access to GPs as necessary.

Evaluate clinical information from examination and history taking and initiate appropriate treatment and / or referral, including admission to hospital if required.

Signpost appropriately to other services in the identified healthcare pathway and to refer patients to appropriate care settings, within locally agreed healthcare pathways.

Maintain accurate and systematic documentation of patient interactions and clinical care provided, including adverse events.

Advise and support members of the public on healthy lifestyles, health promotion and disease prevention.

Provide clinical advice to other members of the team where appropriate.

Support the GPs in other duties as required for example assisting with ECGs or other nursing duties where a nurse is not available.

Responsibilities underpinning the role

To develop and facilitate a good working relationship with practice and community teams.

To plan and organise own workload, and to support colleagues as well as patients and carers.

To enrol in, undertake or qualify from appropriate training as set out by the Personalised Care Institute.

To attend meetings of relevance as agreed with the post holders line manager.

To undertake any other duties commensurate with the post holders grade as agreed with the post holders line manager.

Professional

Input to and extract information from clinical IT system.

Work collaboratively with other members of the health care team such as the, staff in Care Homes, Ambulance Service, and Secondary Care etc.

Ensure that the wellbeing of patients and staff is maintained in accordance with Practice policies. To undertake clinical audits in line with the organisations clinical audit plan, and to participate as required in equipment testing and other research.

Contribute to the analysis of information to inform future development of the service.

Participate and contribute to clinical meetings

Identify risks to service provision and highlight them to the Clinical Lead.

Work within the requirements of Data Protection legislation as it applies to patient records.

Adhere to HCPC Code of Professional Conduct and at all times practice within the scope of professional practice

Person Specification

Experience

Essential

• Able to provide clear and useful guidance and reports on complex matters to stakeholders at all levels
• Evidence of excellent clinical record keeping
• Ability to support patients during acute episodes of illness and to self-manage long term conditions where feasible.
• Evidence of excellent communication skills including diplomatic and negotiation skills
• Able to work under pressure and to meet deadlines
• Able to articulate personal development needs
• Ability to reflect & appraise own performance
• Ability to relate to people from differing backgrounds

Desirable

• Experience of working in Primary care.
• Experience of working with vulnerable patients.

Qualifications

Essential

• Registered Paramedic with HCPC
• Minimum of three years post registration with demonstrable experience of using diagnostic skills
• Experience of dealing with vulnerable patients

Desirable

• Paramedic - prescribing qualification desirable
• Advanced Clinical Practitioner/Advanced Nurse Practitioner
• Demonstrates accountability for delivering professional expertise and direct service provision
• Evidence of working in a community/primary care setting
• Experience in teaching & supporting junior staff/students
