Job summary
To work as an autonomous practitioner, responsible for the provision of medical services to the practice population, delivering an excellent standard of clinical care whilst complying with the PMS contract and other contractual and regulatory obligations. The post-holder will adhere to the GMC standards for good medical practice, contributing to the effective management of the practice, leading by example, maintaining a positive, collaborative working relationship with the multidisciplinary team
Main duties of the job
Make professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
Assessing the health care needs of patients with undifferentiated and undiagnosed problems
Screening patients for disease risk factors and early signs of illness
Developing Care Plans in consultation with patients and in line with current disease management protocols
Conduct reviews of patients placed on the Practice Case Management List
Providing health education
Referring to other care providers as appropriate
Recording clear and contemporaneous consultation notes to agreed standards
Collecting data for audit purposes
Compiling and issuing computer-generated acute prescriptions (avoiding hand- written prescriptions whenever possible)
Prescribing in accordance with the local Medicines Management Board prescribing formulary (or generically) whenever this is clinically appropriate
Referrals should be completed on a daily basis
Results are normally communicated either electronically or by letter and you will be expected to action these on a daily basis for your own patients and on behalf of absent clinicians.
The daily post is shared out
You will be expected to complete examinations and reports for the Benefits Agency and those required under the PMS contract

We provide joint injections, family planning services, smoking cessation clinics etc.
We have a strong training ethos and take pride in training the primary care clinicians of the future.
We offer a competitive salary commensurate with experience, along with 6 weeks annual leave (pro-rata) plus protected time for CPD, quality of life is very important to us as a team.
Post holders will be eligible to join the NHS Pension scheme.
