Job summary
We are
looking for an Advanced Nurse Practitioner/ Advanced Practitioner 37.5 hours per week, can be worked over 4 days, to join our
friendly and professional forward-thinking practice team. A highly skilled clinician who wants to make
a difference to the health and lives of the patients we care for. You will already be an experienced clinician
working autonomously in General Practice, working within your own professional
clinical competencies. You will have the
peer-to-peer support of your colleagues.
Main duties of the job
You
will be required to undertake daily online and face to face appointments for
both acute problems and more complex problems, dealing with patients with
co-morbidities, in addition to patient triage and care navigation. RCN or HCPC is
required as well as Advanced Qualification and independent prescribing
qualifications and be competent in patient assessment and triage.
Process your own pathology results and docman letters regarding patients within your care.

We are
a 4-partner practice, with 4 Salaried GPs and a team of 3 Advanced
Practitioners. We have a large nursing
team consisting of 3 Practice Nurses and 3 Health Care Assistants. We have 2
Practice employed Pharmacists and the wider Primary Care Network team who
support the Practice.

a high achieving practice with regards to QOF and are rated Good by CQC.

Job responsibilities
The
following are the core responsibilities of the advanced (nurse) practitioner. There
may be on occasion, a requirement to carry out other tasks; this will be
dependent upon factors such as workload and staffing levels:
- Manage
patients presenting with a range of acute and chronic medical conditions,
providing subject matter and advice to patients
- Provide
advanced, specialist care to patients as required in accordance with clinical
based evidence, NICE and the NSF
- c.
Request
pathology services as necessary
- d.
Process
and interpret pathology and other test results as required
- e.
Maintain
accurate clinical records in conjunction with extant legislation
- f.
Ensure
read codes are used effectively
- g.
Maintain
chronic disease registers
- h.
Prioritise
health issues and intervene appropriately
- i.
Support
the team in dealing with clinical emergencies
- j.
Recognise,
assess, and refer patients presenting with mental health needs
- k.
Extended
and supplementary prescriber, adhering to extant guidance
- l.
Support
patients in the use of their prescribed medicines or over the counter medicines
(within own scope of practice), reviewing annually as required
- m.
Contribute
to practice targets (QOF etc.), complying with local and regional guidance
- n.
Liaise
with external services / agencies to ensure the patient is supported
appropriately (vulnerable patients etc.)

Delegate
clinical responsibilities appropriately (ensuring safe practice and the task is
within the scope of practice of the individual)
- p.
Support
the clinical team with all safeguarding matters, in accordance with local and
national policies
- Understand practice and local policies for
substance abuse and addictive behaviour, referring patients appropriately.
In
addition to the primary responsibilities, the advanced nurse practitioner
advanced practitioner may be requested to:

Participate
in local initiatives to enhance service delivery and patient care
- b.
Support
and participate in shared learning within the practice
- c.
Develops
an area of specialist interest, taking the lead within the practice
- d. Continually review
clinical practices, responding to national policies and initiatives where
appropriate
- e. Participate in the
review of significant and near-miss events applying a structured approach i.e.
root cause analysis (RCA)
- f. Drives the development
of services within the practice, liaising external agencies and professional
organisations as required
- Develops practice administrative and clinical
protocols in line with the needs of the patient and current legislation
