Job summary
Old Swan Health Centre Group Practice is looking to recruit an enthusiastic and motivated Practice Nurse to join our busy but friendly team serving 9,300 patients. The successful candidate will have excellent clinical and communication skills, a caring nature who will provide and maintain a professional manner and high standard of nursing care.
Main duties of the job
We are looking for an experienced practice nurse who is confident in chronic disease management and puts patient care at the centre of what they do. The successful candidate will have excellent organisational skills and works well within a team.
The candidate will work as part of a team in developing primary care services for our Practice population, providing assessments, screening, treatment, health education advice including chronic disease management, cervical screening, immunisations and health promotion independently or within agreed protocols.

Our team includes 5 partners, 3 salaried GPs, 4 Registrars, ANP, 2 Practice Nurses, HCA, Pharmacist, Physiotherapist, Practice Manager, Reception Manager, Secretary and a team of Receptionist/Care Navigators and Administrative Staff.

1. Providing assessment, screening and treatment

2. Developing and organising with others, nurse-led
clinics for chronic disease management programmes within the practices, in line with national and local policies:
Asthma
Coronary Heart

including baby and travel immunisations
