Health Partners At Violet Melchett are looking for a practice nurse for
our busy 10,000 patient GP practice. Alongside traditional GP services we offer
a range of community services and activities so your job will be busy and
varied. You will be based at our newly refurbished premises just off the Kings
Road, Chelsea SW3 .
Practice Nurse Job Description
Responsible to: Clinically the Partners/Administratively the Business manager.
Clinical Duties & Key Responsibilities
The practice nursing team leads and contributes to the following areas.
There is the scope for specialisation, though all nurses need to provide core
nursing services:
Core nursing services to provide all services
Health promotion activities, such as smoking cessation, weight
management, dietary and exercise advice
Sexual health and contraception
Cervical cytology
Childhood immunisations
Travel health
Blood taking
Ear syringing
Assess and care for patients presenting with wounds
Removal of all types of skin closure materials
See patients for urgent and opportunistic nursing care
New patient checks
NHS health checks (vascular risk)
Demonstrate and teach patients, as appropriate, principles of care and
procedures, working towards their independence and self-care
Ambulatory BP
Administer drugs and vaccines by any route, according to NMC, practice
and, where appropriate the NHS England or CCG guidelines
Extended role of nurses to offer a range of services from this list
Long Term Conditions management Including annual reviews:
Diabetes, asthma and COPD, hypertension and heart disease
Anti-coagulation, First contact, Minor ailments and ECGs
Clinical organisation
Assisting the HCAs in ordering and stock control to ensure the clinical
rooms are adequately stocked
Assisting the HCAs with the organisation of clinical wastes e.g. sharps bins. Working on health campaigns and waiting room displays
In collaboration with practice staff, to maintain clerical organisation and
documentation of the population at risk, and instituting appropriate recall
systems.
Ensure CQC compliance in all activities related to the role.
Management of Risk
Monitor work areas and practices to ensure they are safe and free from
hazards and conform to health, safety and security legislation, policies,
procedures and guidelines
Ensure safe storage, rotation and disposal of vaccines and drugs is
undertaken.
Monitoring fridge temperatures
Ensure the adequate stocking and monitoring of emergency drugs and
equipment.
Where appropriate, oversee the monitoring, stock control and documentation
of controlled drug usage according to legal requirements
Apply infection control measures within the practice according to local
and national guidelines
Be aware of and adhere to all statutory provisions, Health and Safety
Regulations and NMC code of conduct.
Ensure practice policies and NHSE policies - where they apply to the
practice - are followed
Communication
To attend and participate in practice meetings and discussions, for
example on the development of clinical protocols and the delivery of enhanced
services.
To attend and participate in practice nursing team meetings, network
MDTs and learning events both within the practice and at a borough level.
To discuss with the practice manager and partners as appropriate any
factors affecting the delivery of care and agree appropriate action.
Team Working
Understand own role and scope in the organisation and identify how this
may develop over time
Delegate clearly and appropriately, adopting the principles of safe
practice and assessment of competence of those taking on delegated duties
Ensure clear understanding and utilisation of referral mechanisms within
the practice
Accept delegation from other team members where appropriate, prioritise
own workload and ensure effective time-management strategies are embedded in
own practice
Assist in the formulation of practice philosophy, strategy and policy
and participate in the administrative and professional responsibilities of the
practice team in providing primary care services.
Initiate, develop, implement and evaluate pathways of care and health
promotion programmes in collaboration with other members of the primary care team,
clinical and administrative staff.
The post holder will be supported by, and give support to, others in the
nursing team.
Utilising Information
Keep accurate up-to-date records of clinical and administrative work and
conduct regular audits of work, including participating in research as agreed
by the practice.
Run clinical searches using Systm1 to ensure recall for smears and LTCs
are up-to-date.
Learning and Development
Maintain your own clinical expertise and attend relevant study sessions.
Training needs include the responsibility to ensure the post holder contributes
to the effectiveness of the practice team as a whole
Undertake procedures regarded as the extended role of the nurse for
which specific experience or training will be needed and authorisation is given
by the partnership
Act as mentor to the other members of the nursing team, assessing
competence against set standards as requested and if appropriately qualified
Disseminate learning and information gained to other team members in
order to share good practice and inform others about current and future
developments
Undertake mandatory and statutory training
