Were a small
(3000 patient), friendly, two partner, semi-rural training practice in
Clun, Shropshire. Were looking for a 4 session per week, newly qualified GP to
replace our current, much appreciated salaried GP who is sadly (for us) heading
off to work in Australia.
We offer a
supportive work environment and are very happy to be flexible with adapting
working hours to your needs. The local population is very different from that
in an urban setting, with a large number of farming families with histories in
the area stretching back for generations. Theyre a pretty hardy lot and we
have a very low home visiting rate.
Clun is an
easy drive from Shrewsbury (where our Practice Manager lives), Telford (where
our current salaried GP lives) and the area is often cited as one of the best
parts of the UK to live.

Main duties of the job
Provision of General Medical Services, Enhanced Services, and work with the Primary Care Network, in accordance with the GMS and other practice contracts. Continuing professional development opportunities, and regular practice meetings.
About us
The Meadows has 2 GP Partners and 1 Salaried GPs.
We have two experienced practice nurses, one Health Care Assistant and a phlebotomist. As part of the South West Shropshire Primary Care Network we are also supported by a First Contact Physiotherapist and a Clinical Pharmacist as well as a Social Prescriber.
We are a well-established team with a low turnover of staff. We welcome prospective applicants in for an informal chat with the clinical team, admin team, and a tour of the Practice.

Main Duties & Responsibilities
Clinical Responsibilities
- In accordance with the Practice timetable, as agreed, the post-holder will be available to undertake a variety of duties including surgery consultations, telephone consultations, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork, reports and correspondence in a timely fashion
- Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
- Assessing the health care needs of patients with undifferentiated and undiagnosed problems
- Screening patients for disease risk factors and early signs of illness
- In consultation with patients and in line with current Practice disease management protocols, developing care plans for health and promoting healthy lifestyle strategies and education
- Manage long-term conditions and patients with complex problems
- Recording clear and contemporaneous consultation notes to agreed standards
- Collecting data for audit purposes and complying with QOF requirements
- Delivering enhanced services
- Check, manage & process patient test results
- Refer patients to secondary care and/or relevant organisations and internally within the PCN (eg to additional roles members of the PCN team) as required
- Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible) in line with local and national guidelines
- Prescribing in accordance with the Practice prescribing formulary (or generically) whenever this is clinically appropriate
- In general the post-holder will be expected to undertake the duties and responsibilities associated with a GP working within primary care in a surgery on a PCN basis
