Job summary

We are looking for a friendly and motivated care co-ordinator with a specific interest in medication management to join our supportive GP practice team.

This role combines the responsibilities of a Care Co-ordinator and a Pharmacy Assistant, focusing on delivering personalised, patient-centred care and supporting the pharmacy team in medication-related services. The postholder will work closely with clinical and non-clinical staff to ensure patientsparticularly those with complex needsreceive coordinated care and appropriate medication support.

This is a patient-facing role, designed to support both the pharmacy team within the GP practice and the wider local community.

Main duties of the job

  • Help patients understand their medications, including dosage and side effects.
  • Complete basic medication reviews and manage patients long term conditions under pharmacist supervision.
  • Deliver patient-facing and supporting roles to ensure safe, effective medicines use through shared decision-making.
  • Undertake medicines optimisation tasks such as checking inhaler technique, and supporting medicines reconciliation
  • Perform administrative duties related to pharmacy operations.
  • Support in the supervision of reception and prescription teams in sorting prescription requests and medication related queries
  • Provide specialist expertise (where competent) on public health and social care needs, including lifestyle advice, service information, and tackling health inequalities.

Experience

  • Strong communication and relationship-building skills.

Experience of developing relationships with a wide variety of people

Desirable:

  • Experience in pharmacy or medicines-related work.
  • Experience in inpatient environments or ward-based settings.
  • Experience in GP or primary care environments
