Job summary

Were looking for an experienced ACP to join our primary care team, delivering high-quality, person-centred care to residents in care homes and housebound patients. Youll assess, diagnose, prescribe, and manage frailty and long-term conditions, working closely with GPs, pharmacists, nurses, and care staff to improve outcomes and reduce hospital admissions.

You will be working within the Enhanced Health in Care Homes (EHCH) model supporting people living in care homes who can expect to receive the same level of support as if they were living in their own home.

Supporting our housebound patients to improve the management of LTCs and help reduce admissions.

Triaging and attending acute home visits in rotation with the clinical team.

Our aim is working together to provide high quality, accessible, patient focused and sustainable healthcare.

Main duties of the job

Working with our existing clinicians to support the work of the GPs in the practice with care-home visits and ward-rounds, housebound patients and acute home visiting.

Weekly care home rounds, liaising closely with care home staff, practice GPs and colleagues in support of their residents.

Occasionally support the GPs by triaging and attending acute home visits in rotation with the established clinical team. To manage acute illnesses in patients who are not able to attend the surgery, may include arranging admissions to hospital or liaising with community teams, palliative care teams and social care.

Delivering support to our housebound patients. To improve management of LTCs, help with decisions around ceilings of treatment and reduce hospital admissions.

To deliver a high standard of patient care, using advanced autonomous clinical skills, and a broad in-depth theoretical knowledge base.

To contribute to the QOF/IIF targets for the Practices and support the ongoing development of both the care home and visiting service.

About us

The Merchiston practice was established around 1860 in the village of Stratton-St-Margaret. Until the 1950s, Stratton was a small farming community and it still retains its separate identity, despite now being a suburb of Swindon. The Partnership moved from the original Doctors house into new purpose-built premises in 1992.

Merchiston Surgery is the larger Practice with a raw patient list of 13,500 (13,700 weighted list)

We are a teaching practice with 2 Education Supervisors, 2 Clinical Supervisor, supporting 3 Trainee GPs. Rated excellent by the Swindon GP QA Panel for our trainee placement.

Following a recent inspection we are rated Good across all domains by the CQC.

Communicate effectively with patients and carers/care home staff (nurses/carers), recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background, and preferred ways of communicating.

Recognising issues and gathering sufficient information to refer (e.g. drugs; domestic violence; child protection; vulnerable adults; senior patients; social problems).

Manage and assess risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients.

Be mindful of work areas and practices to ensure they are safe and free from hazards and conform to health, safety and security legislation, policies, procedures and guidelines.

Follow all necessary infection-control measures according to local and national guidelines.
