Job summary

We are looking for a motivated and compassionate Care Co-ordinator to join our team and support the delivery of high-quality, personalised care for our patients. Working within one GP practice in our PCN (North Dartmoor), you will play a key role in helping patients navigate the health and care system, ensuring they receive the right support at the right time.

This is an excellent opportunity for someone who is organised, approachable, and passionate about improving patient experience. You will work closely with clinicians, practice manager and admin and nurse teams to support proactive care, care planning, and seamless coordination across services.

This role could be a job share for the right candidates.

Main duties of the job

  • Act as the central point of contact for patients requiring care coordination and personalised care support.
  • Support the development and delivery of personalised care and support plans.
  • Assist in identifying and managing patients who may benefit from targeted support or proactive care initiatives.
  • Work closely with multidisciplinary teams to ensure smooth communication and continuity of care.
  • Maintain accurate, timely data and contribute to reporting and ongoing service improvement.
  • Build trusted relationships with patients, helping them feel supported and informed throughout their care journey.
  • Manage and co-ordinate NHS campaigns and initiatives ensuring all eligible patients are targeted.

  • Use of digital tools to maintain practice website and Facebook page

You will be joining a small, close-knit practice whose team are supportive and forward thinking. Working with us will mean that you have a dedicated (and fun) clinical and admin team ready and willing to support you with the addition of 2 Physios, a Clinical Pharmacist and an ACP.

RESPONSIBLE TO: Practice Manager

OVERVIEW

This is a role established to work with vulnerable patients from our practice location, both remotely and in person. You will be working closely with our Health Co-ordinator team and clinicians to support the wellbeing and care needs of our patient population. You will have access to general advice and support and will also have clinical team oversight or supervision from a Partner where necessary.

MAIN PURPOSES

  • To provide wide-ranging, proactive and efficient administrative support to Cheriton Bishop & Teign Valley Practice
  • To be responsible for the administration, co-ordination and monitoring of dedicated or targeted clinical campaigns
  • To work closely with Practice Manager, nursing and coding teams to assist the practice in meeting contractual targets
  • To utilise population health intelligence to proactively identify and work with a cohort of patients to deliver personalised care, including through recalls for specific patient groups (eg, immunisations, health checks, reviews etc)
  • To discuss patient-related concerns with the relevant staff and be supported to follow appropriate safeguarding procedures
  • To holistically bring together all of a persons identified care and support needs and explore options to meet these within a single personalised care and support plan (PCSP), in line with PCSP best practice, based on what matters to the person
  • To help people manage their needs through answering queries, making and managing appointments and ensuring that people have good quality written or verbal information to help them make choices about their care
  • To provide co-ordination and navigation for people and their carers across health and care services, working closely with other primary care professionals.
  • To work with GPs and other primary care colleagues to identify and manage a caseload of patients, and where required and as appropriate, refer people back to other health professionals

The post holder will be aware of the various statutory requirements and will assist in implementation as directed by the practice manager. There is a need to work closely with Health Co-ordinators and clinical staff to ensure the smooth running of the practice, reporting any problems encountered to the relevant person.

Training requirements will be monitored by annual appraisal. Personal development will be encouraged and supported by the practice. It is the individuals responsibility to remain up to date with recent developments.

PRINCIPAL DUTIES AND RESPONSIBILITIES

The post-holder will assist in promoting and maintaining their own and others health, safety and security as defined in the practice Health & Safety policy, the practice Health & Safety manual, and the practice Infection Control policy and published procedures. This will include:

  • Using personal security systems within the workplace according to practice guidelines

  • Making effective use of training to update knowledge and skills
  • Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and keeping them free from hazards
  • Actively reporting health and safety hazards and infection hazards immediately when recognised
  • Keeping own work areas and general / patient areas generally clean, assisting in the maintenance of general standards of cleanliness consistent with the scope of the job holders role
