Job summary

Cheriton Bishop & Teign Valley Practice, part of North Dartmoor PCN, are seeking a skill Paramedic or Advanced Nurse Practitioner with a passion for delivering high-quality, patient-centred cared. We are looking for someone who has experience of working within primary care.

Main duties of the job

The role will entail carrying out acute assessments and the management of Long Term Conditions both in practice and in the Patient's home. Being qualified as a non-medical Prescriber would be desirable but not essential.

The role is for 18 hours per week and we would ideally like these to be worked over 3 days, 9am to 3pm.

Advanced Clinical Practitioner Paramedic / Advanced Nurse Practitioner

RESPONSIBLE TOGP Partners Clinically, Practice Manager - Administratively

OVERVIEW

This is a role established to work with vulnerable patients from our practice location, both remotely and in person. You will be required to deliver a high standard of patient care within your scope of practice to meet the needs of the Practice patient population and to deal with presenting patients needs in a Primary Care setting.

MAIN PURPOSES

Provide first point of contact for patients presenting with undiagnosed problems, using history taking, physical examination, problem solving and clinical decision making skills to reach a working diagnosis and management plan in partnership with patients and their carers.

Assess and triage patients, including same day triage and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways.

Undertake Practice based consultations for emergency or routine problems including management of exacerbations of long term conditions.

Undertake telephone consultations, clinics, home visits and participate in our same day service during Practice opening hours.

Instigate diagnostic tests or investigations in Primary Care and interpret findings/reports within the scope of your practice, discussing the results and implications with your patients

Utilise clinical guidelines and promote evidence based practice

Refer patients directly to other services using appropriate referral pathways

Provide preventative healthcare and advice

Provide medical support as required to patients

Ensure safe handover of care within and outside the Practice as appropriate

Work directly with members of the Practice team and support integrated patient centred care working with wider Primary Care / Community Care / Social Care networks

Identify Community Health needs and participate in the development of patient/family centred strategies to address them

Contribute to the Practice quality targets to consistently achieve high standards of safe, evidence based, cost effective patient care and service delivery

Act upon alerts and reminders during consultations

Training requirements will be monitored by annual appraisal. Personal development will be encouraged and supported by the practice. It is the individuals responsibility to remain up to date with recent developments.

PRINCIPAL DUTIES AND RESPONSIBILITIES

Health & safety

The post-holder will assist in promoting and maintaining their own and others health, safety and security as defined in the practice Health & Safety policy, the practice Health & Safety manual, and the practice Infection Control policy and published procedures. This will include

  • Using personal security systems within the workplace according to practice guidelines
  • Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks
  • Making effective use of training to update knowledge and skills
  • Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and keeping them free from hazards
  • Actively reporting health and safety hazards and infection hazards immediately when recognised
  • Keeping own work areas and general / patient areas generally clean, assisting in the maintenance of general standards of cleanliness consistent with the scope of the job holders role
  • Undertaking periodic infection control training (minimum annually)
  • Reporting potential risks identified

Production of performance and quality information

To ensure that documentation (electronic and paper-based) exists to support performance standards across the full range of performance-based activity QOF, Enhanced Services etc.

To assist in the production of information for clinical audit.

To undertake ad-hoc work related to the performance of the practice under the guidance of the practice manager.

Data quality

To work within the clinical computer system to improve data quality, using the expertise of other data staff where appropriate.
