Main Duties and
Responsibilities
To
provide a complementary source of care to that of the Independent Nurse
Practitioners and GPs.
To

Work
autonomously across role boundaries to provide alternative comprehensive
healthcare, sensitive to the needs of the population.
To
take a leading role in developing services to meet the needs of the Contract.
This will necessitate multi-site working.
Clinical
To see patients that present with
previously undifferentiated, undiagnosed problems, taking responsibility for
their health assessment and physical examination, the diagnosis of their
condition and its management or referral to another member of the
multidisciplinary team deemed appropriate by the practitioner within their
professional competencies.
To make differential diagnosis using
decision making and problem solving skills.
Makes professionally autonomous decisions
for which he or she is accountable.
Orders necessary investigations and
provides treatment and care both individually, as part of a team, and through
direct referral to other primary and secondary agencies.
To provide clinical leadership.
The planning, implementation and
evaluation of the chronic disease management programmes in line with current
evidence based best practice and the GMS contract, reducing unnecessary
hospital admissions and promoting the preventative aspect of nursing practice.
Prescribe as appropriate using the
practice formulary and as an Independent Nurse Prescriber.
Screens patients for disease risk factors
and early signs of illness following NICE guidelines.
The management plan to include health
promotion, health protection and disease prevention as appropriate.
To admit and discharge patients from their
caseload.
To record all information accurately in
conjunction with practice policy and current professional guidelines.
Research
Monitor
research in order to improve quality care and disseminate research findings.
To
ensure evidence based practice personally and throughout the nursing team.
To
undertake an active role in research and audit to better improve patient
outcomes.
Participate
in the Clinical Governance agenda at practice level.
Management
To
work to the NMC Code of Professional Conduct.
To
provide a leadership and consultancy function as required.
To
make a significant contribution to the development and direction of the
practice.
To
participate in multidisciplinary protocol development.
To
participate in staff appraisal.
To
promote communication and provide learning opportunities for other staff.
Professional Education, Training and
Development
Maintain
professional development in line with Post Registration Education and Practice
(PREP) and NMC Revalidation requirements.
To
maintain a professional portfolio and actively keep up to date with the latest
developments and guidelines for best practice and disseminate this information
accordingly.
To
encourage, facilitate and participate in clinical supervision within the
practice.
To
be a resource for advice and guidance for other members of the team.
Communication
To
work collaborately and in partnership with all other members of the
multidisciplinary team.
Health and Safety
To conform to Health and Safety
Regulations in line with Primary Care
Policy
to allow manual handling in the event of this requirement arising within the
surgery.
To
have an understanding of Risk Assessment, Risk Management and
Hazard Management in line with COSHH regulations.
To report any accident of incident to Nurse
Practitioners.
