Job summary

We are a GP Practice in Swindon, looking to recruit an enthusiastic and motivated Nurse Practitioner. We have 3 surgery sites in Swindon and you will be required to work at all sites.With a growing practice population, we are keen to continue to develop our clinical team.

Main duties of the job

We are looking for someone with excellent communication skills and a commitment to working as part of a strong and effective clinical team. We are flexible in terms of hours and a competitive salary will be offered to reflect the successful candidates experience and qualifications.

Whalebridge Practice is looking for a Practice Nurse to join their multi-disciplinary team, to work over 3 sites within Swindon.

The role can be part time or full time and the rate of remuneration will be based on qualifications and experience.

Job Title: Nurse Practitioner

Grade: Band 7 equivalent

Reporting to: Nurse Manager

Job Summary/Main Purpose

This multi-site position is that of a Nurse Practitioner within the Whalebridge Practice. The practice serves the population of Central and West Swindon and offers services across three sites within Swindon. We offer a wide range of clinics and appointments to include chronic disease management, cervical cytology, immunisations of babies and adults, and contraceptive services across several sites.

Principal Duties

To see, diagnose and treat patients with minor ailments.

To provide assessment and clinical decision making using skilled evidence based knowledge working without supervision.

To carry out home visits to patients who are acutely unwell.

To carry out e-consultations, using the PATCHs system.

Individual Responsibilities

To assess, make clinical decisions and treat, using skilled evidence based nursing knowledge

To use effective communication in all situations which may necessitate the use of a telephone interpretation service

To refer patients to other health care professionals as appropriate and necessary

To provide advice on screening and health promotion at every opportunity

To record all information accurately in conjunction with local policy and current professional guidelines

To work in partnership with other members of the multidisciplinary team

To supervise junior members of the team

To mentor students during placements, facilitate programmes for learners

To maintain clinical/professional expertise to a high standard in accordance with NMC Code of Conduct

Extend professional practice and competencies

To participate in staff appraisal

To act ethically to meet the needs of the patient

Education and Development

To participate in staff development, personal development planning and clinical supervision

To take part in the appraisal and revalidation processes.

Participate in Research programmes and explore opportunities undertake research to enhance the body of knowledge in this field of nursing

Assist with audit programmes

To attend training to develop and maintain skill.

To keep registration up to date

To assist in the development and implementation of policies/procedures and guidance relating to nursing practice

To keep abreast of professional developments, both locally and nationally

You must be aware of the Caldicott Principles as they affect your work. You can address any questions in relation to this to your line manager.
