Job summary

Rawmarsh Health Centre has an exciting opportunity for an experienced Care Navigator/ Receptionist to join the team.

We are recruiting for two positions at 16 hours per week. The working hours will be a pattern of morning and afternoon shifts with the opportunity for overtime covering staff annual leave and sickness.

The successful candidate will provide a point of contact for patients and act as a focal point of communications between patients and all member of the Primary Care Team.

Main duties of the job

  • Act as a focal point of communication
  • Be able to cover all reception positions as necessary.
  • Process appointment requests from patients by telephone and in person.
  • Signposting patients across services within Primary Care.
  • Process repeat prescription requests in accordance with Practice guidelines.

About us

Rawmarsh Health Centre is based within Rawmarsh Customer Service Centre on Barbers Avenue in Rawmarsh. We have around 4,000 registered patients.

The team comprises a Partner GP, a salaried GP, a Physician Associate and our nursing team. All are supported by our Care Navigators and administration team.

Duties and Responsibilities:

The duties and responsibilities to be undertaken may include any or all of the items in the following list. Duties may be varied from time to time under the direction of the Reception Supervisor or Practice Manager, dependent on current and evolving practice workload and staffing levels:

  • Maintaining and monitoring the Practice appointments system including using the care navigation pathway
  • Taking messages and passing on information via tasks, notifications or via telephone
  • Computer data entry processing and recording information in accordance with Practice procedures
  • Process prescriptions in accordance with practice guidelines
  • Providing clerical assistance as required, including word/data processing, filing, photocopying and scanning and filing of hospital / medical letters into the patients computerised record.
  • Undertake any other additional duties appropriate to the post as requested by the Partners, Practice Manager or any of the management team.

  • GCSE Mathematics

  • NVQ in Customer Service

Experience

  • Practical experience working in a public reception environment
  • Experience of using own initiative

  • Experience of working in a GP Practice
  • Experience of working in Primary Care
  • Clinical system experience ( EMIS / System One )
