Job summary

Ellison View Surgery is seeking an enthusiastic and compassionate Advanced Nurse Practitioner with experience in Primary Care to join our team on a part-time basis.

The successful candidate will be educated to an advanced level of practice and able to work autonomously, making clinical decisions with confidence while collaborating effectively within a multi-disciplinary team. Operating within our total triage model, you will be responsible for assessing patients promptly, prioritising care appropriately, and ensuring safe and effective management of both acute illness and minor ailments. You will arrange and interpret investigations, make independent referrals, and manage complex clinical situations using your advanced knowledge and critical thinking skills. Your role will encompass comprehensive patient care, including history taking, clinical assessment, diagnosis, treatment planning, patient education, and evaluation of outcomes, always maintaining professional standards and ensuring high-quality, patient-centred care.

The successful candidate will also demonstrate strong communication skills, a compassionate approach to patient care, and the ability to support colleagues in delivering a seamless, efficient service.

Main duties of the job

We are a small practice serving 6,100 patientsin the centre of Hebburn, comprising 3 GP partners,1 salaried GP, 2 experienced practice nurses, 3 healthcare assistants, and a dedicated team of medical administrators. Our practice ethos emphasises collaboration and support, with a focus on delivering high-quality patient care.

Our wider multidisciplinary team includes a social prescriber, pharmacist, first contact physiotherapists, and first contact podiatrists as part of the local Primary Care Network. We are actively involved in the training of F2s, providing an excellent environment for professional development.

Our ANP will be responsible for delivering high-quality health services across the practice, including but not limited to the following

Health Promotion & Wellbeing

  • Develop, implement, and embed health promotion and wellbeing programmes.

Deliver opportunistic health promotion where appropriate

Patient Management & Clinical Care

  • Manage patients presenting with a range of acute and chronic medical conditions, providing expert clinical advice.

  • Implement and evaluate individualised treatment plans for patients with chronic diseases.

  • Identify, manage, and support patients at risk of developing long-term conditions to prevent adverse health outcomes.

  • Provide advanced, specialist nursing care in accordance with evidence-based practice, NICE guidance, and relevant national service frameworks.

  • Recognise, assess, and refer patients presenting with mental health needs.

  • Provide chronic disease clinics, referring patients to secondary/specialist care where necessary.

  • Support patients in the use of prescribed and over-the-counter medicines within your scope of practice, reviewing annually as required.

  • Be an extended and supplementary prescriber, adhering to current guidance.

Clinical Procedures & Diagnostics

  • Request, collect, process, and interpret pathological specimens and other test results as required.

  • Chaperone patients as appropriate.

  • Prioritise health issues and intervene appropriately.

  • Support the team in dealing with clinical emergencies.

  • Provide vaccination programmes for both adults and children

Record Keeping & Administration

  • Maintain accurate clinical records in accordance with legislation, using SNOMED CT codes effectively.

Safeguarding & Professional Responsibilities

  • Adhere to practice policies and procedures on Safeguarding Adults and Safeguarding Children.
  • Understand and implement policies for substance abuse and addictive behaviour, referring patients appropriately.
  • Delegate clinical responsibilities safely and appropriately within the scope of practice of colleagues.
  • Liaise with external services/agencies to ensure vulnerable patients are appropriately supported.

In addition to clinical duties, the ANP will contribute to the wider functioning and development of the practice:

  • Provide holistic support in clinical governance, taking a senior role in ensuring compliance.
  • Support the Infection Prevention and Control (IPC) lead, ensuring practice compliance.
  • Monitor and ensure the safe storage, rotation, and disposal of medications.
  • Provide leadership and management for the nursing team, including newly qualified staff.
  • Participate in local initiatives to enhance service delivery and patient care.
  • Support and contribute to shared learning across the organisation.
  • Develop an area of specialist interest, taking a leadership role where appropriate.
  • Continually review clinical practices, ensuring alignment with national policies and initiatives.
