General Practice Nurse

Job summary

As a Practice Nurse at High Street Medical Practice, you will be responsible for several clinical areas such as health promotion, chronic disease management and ormone replacement therapy.

We are looking for a motivated and compassionate Practice Nurse who enjoys working as part of a team to provide excellent patient care.

You will need to have understanding, acceptance and adherence to the need for strict confidentiality and the ability to work under pressure.

Main duties of the job

Clinical Duties:

• Provide skilled nursing care and health assessments to patients.
• Manage and monitor patients with chronic diseases (e.g., asthma, COPD, diabetes, CKD )
• Administer vaccinations and injections (including childhood immunisations, flu jabs,).
• Conduct cervical smears in line with national screening guidelines.
• Provide contraceptive advice and support (as trained/appropriate).
• Hormone replacement therapy
• Provide advice and support for lifestyle changes, including smoking cessation, weight management, and alcohol use.
• Participate in infection prevention and control measures and maintain clinical cleanliness standards.

Administrative and Team Duties:

• Maintain accurate patient records using the EMIS clinical system.
• Participate in the call/recall system for chronic disease and screening programs.
• Support Quality and Outcomes Framework (QOF) delivery and other enhanced services.
• Contribute to clinical audits and quality improvement initiatives.
• Participate in multidisciplinary team meetings and case discussions.
