Job summary

As part of East Cleveland Group multi-disciplinary team, the post-holder will play a key role in developing and maintaining clinically and cost-effective prescribing provision across the network population - improving patient outcomes and helping to relieve service and financial pressures across the PCN.

The role will include supported progression through the CPPE Primary Care Pharmacy Education Pathway, providing an exciting opportunity to develop knowledge and skills in relation to patient-facing, person-centred and integrated care.

Main duties of the job

The post holder will be responsible for the care management of patients with chronic diseases undertaking clinical medication reviews to support the proactive management of complex polypharmacy patients. This will include, but not be limited to, elderly people in care homes, patients with multiple co-morbidities and those with learning disability and autism.

The post-holder will additionally provide clinical leadership in the delivery of person-centred medicines optimisation, helping to support standardised approaches to care across the PCN and forging strong relationships with both specialist and community pharmacists to promote system-wide medicines optimisation and quality improvement.

The post-holder will provide day-to-day support to practices and will be responsible for supporting and supervising Pharmacy Technicians.

Ideally the post-holder will be a non-medical prescriber though there will be an opportunity to work towards a prescriber qualification following completion of the Pathway and this will be supported by the network as part of continued professional development.

About us

East Cleveland PCN consists of 5 member practices serving a patient population of approximately 42,000. The successful applicant will be expected to work in any of these practices to support with service delivery.

  • Digital skills, including ability to access and competently use clinical systems and electronic transfer of prescriptions

  • Ability to apply effective clinical judgement in the prioritisation and decision-making aspects of the role
  • Full driving licence

Qualifications

Essential

  • Undergraduate degree in pharmacy and registration with the General Pharmaceutical Council
  • Knowledge and awareness of legal, ethical, professional and organisational policies/procedures relevant to the role

Desirable

  • Hold or be working toward an independent prescribing qualification (although this will be offered as part of the role)
  • Qualified or enrolled on CPPE Clinical Pharmacist Training pathway (although this will be offered as part of the role)
  • Knowledge and understanding of Primary Care Network arrangements and their contribution to the NHS 10-Year Plan
  • Knowledge, awareness and experience of brief intervention and referral pathways that provide opportunistic advice and support for preventable illness and/or long-term conditions receptive to lifestyle/pharmacological support

  • Minimum 2 years pharmacist experience

Desirable

  • Experience in conducting medication reviews
