We are a friendly and motivated team looking for a Practice Nurse to join our expanding team. We are an established GMS practice with a list size of 7700 patients. We have a strong reputation for providing excellent patient care in a friendly and supportive environment. We are keen to attract the right candidate to work with us in our practice to deliver outstanding care for our patients.
We are a Deep End Practice which means we are part of a network of Practices with a patient population in socioeconomically deprived areas of the North East and North Cumbria. We are passionate about having a positive impact on reducing health inequalities in our area.
We are excited for an upcoming development for our Practice as we expand the building by an additional floor to allow for exciting developments such as being able to host medical students, host additional Doctors in Training and being able to recruit additional clinical staff members to improve the capacity of our service.

Main duties of the job
The Practice Nurse will support the delivery of high-quality nursing care and contribute to the smooth operation of clinical services. Key responsibilities include:
- Ensuring patient confidentiality and respecting privacy at all times.
- To implement vaccination programmes for adults and children.
- To develop, implement and embed health promotion and wellbeing programmes
- To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing
- As required, to provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF
- Embracing and supporting changes aimed at improving service delivery.
- Providing compassionate, courteous, and respectful care to all.
- Demonstrating competence through the timely and effective completion of assigned tasks.

About us
We are keen to attract the right candidate to work with us in our practice to deliver outstanding care for our patients. Our team is currently made up of 3 GP Partners, 6 Salaried GPs, 1 Clinical Pharmacists, 2 Practice Managers, 1 Nurse Prescriber and 1 Healthcare Assistant, 2 Care Coordinators and a Safeguarding Care Coordinator with strong support from a motivated clerical team.

Job responsibilities
- Perform cervical screening in line with national guidance.
- Liaise with local services and attend mandatory training.
- Conduct regular audits and provide advice on contraception, HRT, and womens health.
- Support patient education and monitor related supplies.
- Monitor cleanliness and compliance across clinical areas.
- Manage infection prevention measures and ensure supply levels are maintained.
- Process samples and oversee vaccine storage, cold chain, and stock control.
- Support the provision of travel health services.
- Ensure adherence to the practice chaperone policy.
- Monitor patient compliance and assess treatment efficacy.
- Recognise clinical abnormalities and recommend adjustments as appropriate.
- Provide care aligned with NICE and NSF guidance.
- Prescribe medications under PGDs within scope of practice.
- Appropriately delegate tasks ensuring safe practice.
- Assess and manage both routine and complex wounds.
- Use diagnostic tools (e.g. Doppler) as required.
- Support the team in managing clinical emergencies.
- Conduct health assessments and identify lifestyle-related risk factors.
- Develop and evaluate personalised care plans.
- Promote medication adherence and provide education on use and side effects.
- Lead chronic disease clinics and refer to secondary care when necessary.
