Job summary

We are looking for a dynamic nurse practitioner to join our friendly training practice, The Endeavour, based in the centre of Middlesbrough.

Main duties of the job

Investigate and interpret diagnostic investigations as appropriate, including checking patient results for other clinicians.

Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients attending with an undifferentiated diagnosis.

Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly.

Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs.

Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).

Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.

Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs.

Work with patients in order to support compliance with and adherence to prescribed treatments.

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care.

About us

The Ironopolis Medical Group was formed through the administrative merger of Martonside Medical Centre (7900 patients) and The Endeavour Practice (9300 patients) in June 2017.

This position is for The Endeavour Practice. The group has 7 GP partners and a management partner. Both sites are training practices.

We also specialise in Occupational Health and our management partner is the non-clinical chair of Central Middlesbrough PCN. Our clinical system is SystmOne.

Recognise and work within own competence and professional code of conduct as regulated by the NMC.

Maintaining Patient Records / Patient Correspondence

In accordance with the NMC Record Keeping Standards provide full and accurate documentation of the nursing assessment of need and delivery of care.

In accordance with the NMC formulate individual care plans and evaluate outcomes at appropriate levels according to need.

To evaluate the outcome of that care, making changes to the care plan for individual patients where appropriate.

To demonstrate the rationale behind the various methods chosen to meet care needs of patients, to evaluate those methods and change them as necessary.

Practice Development / Quality Standards

Deliver evidence-based care and car according to NICE guidance.

Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation.

Utilise the audit cycle as a means of evaluating the quality of your own work and that of your immediate team, implementing improvements where required.

Create clear referral mechanisms to meet patient need.

Discuss, highlight and work with the team to create opportunities to improve patient care.

Use a structured framework to manage, review and identify learning from patient complaints, clinical incidents and near-miss events.

To be involved in clinical policy development within the practice, providing nursing input to any decision making processes affecting the care of patients.

To contribute to teaching of learners in the practice e.g. medical students about care delivery from the perspective of an experienced nurse.

Delegate clearly and appropriately, adopting the principles of safe practice and assessment of competence.

Manage and assess risk within the areas of responsibility, ensuring adequate measures are in place to protect staff and patients.

Monitor work areas and practices to ensure they are safe and free from hazards and conform to health, safety and security legislation, policies, procedures and guidelines.

Apply infection-control measures within the practice according to local and national guidelines.

General Responsibilities
