At Fareham & Portchester PCN we have an excellent opportunity for a dynamic and forward-thinking person to join our well establish existing team of GPAs, Nurses, ANP and Paramedics. This will involve working across the four practices in the Fareham & Portchester area.
Hours of Work: 25-30 hours per week.
This role is best suited to a HCA or qualified General Practice Assistant, with a proficiency in key areas such as basic clinical duties, administration, communication and record keeping. You must have an understanding of patient centred care with a caring and understanding manor.
Main duties of the job
Working within the GP resilience framework, along side practice staff to provide proactive care, using digital tools to perform geriatric assessments and Incorporating integrated neighbour working
Working in line with the EHCH (Enhance health in care homes) Framework. This will include new resident reviews, personalised care and support plans, and post hospital discharge reviews within the care home setting.
Assist the HVS with Seriously Mentally Impaired and Learning disability reviews, chronic disease and long-term condition reviews
Liaise with patients and PCN staff including pharmacy, social prescriber, MSK HVS and paramedic triage teams regarding the booking of appointments, referrals, tests and follow ups
Dependant on clinical skills undertake dipping urine, blood pressures, ECGs and phlebotomy
Data administration and QOF/IIF target requirements and contract service requirements
To assist in patient, care navigation/co-ordination and the running of the HVS team.
Helping the HVS liaise with outside agencies e.g. getting an on call doctor on the phone to ask advice or arrange admission while the HVS can continue with their consultation(s)
Provide administrative support and delivery of vaccine to patients during Covid and Flu seasons and other PCN administration

The PCN was formed in response to the NHS Long Term Plan and the 2019 GP contract, which emphasized the importance of Primary Care Networks. These networks aim to integrate GP practices with other local servicessuch as community health, mental health, social care, and voluntary organizationsto provide coordinated, proactive, and personalized care for local populations
