Job summary

This is an exciting opportunity for a motivated and enthusiastic person to join our expanding PCN team. This role will be to provide administrative support and services to all members of the Primary Care Network (PCN) and Home Visiting Service

Main duties of the job

This role will be to provide administrative support directly to the management team, and all nonclinical and clinical teams based in the PCN on a daily basis. This will involve Social Prescribers, Clinical Pharmacists, MSK practitioners, and the Paramedics and Nurses from the home visiting team.

You will assist with rotas and clinic building on our EMIS system for our teams and provide administrative support for the Enhanced Access Service. Ensuring the efficient delivery of health care services.

You will be part of our first point of contact for our Home visiting service providing administrative support to our clinicians out on the road, as well as being a first point of contact for incoming staff queries relating to our services. Processing referrals, workflow tasks, and phoning patients to book or cancel appointments for Visits and vaccinations.

You will also provide direct support to the Senior PCN manager and wider management team, to assist with the processing of the management mailbox, information gathering & event organising, Coding for IIF and QoF, stock ordering, Minute taking, and upkeep of spreadsheets and office protocols.

There will also be a requirement to speak with our patients to support the booking of appointments to meet their medical needs.

We are looking for someone to join our expanding Primary Care Network team. We consist of 4 practices from within the Fareham & Portchester area. We are committed to safeguarding our patients and vulnerable people. We have a patient population of approximately 45,500 with a rapidly expanding PCN. All 4 practices are training-based and we pride ourselves on the development of our in-house supervision structure for all of our ARRS roles within the PCN, and have a strong ethos in the development and training of our staff. Job Summary:

Provide continued administrative support of the services delivered by the FPPCN. In particular, providing direct support to the PCN Manager and Home Visiting Service (HVS).

PCN Admin Support:

To provide direct administrative support to the PCN Manager

To assist Team leads, clinical and non-clinical with administrative support, for data entry, coding of EMIS records, auditing and routine reports, texting patients, or booking appointments, telephone calls to patients, general office duties and inquiries when required

Act as the second point of contact for support to all PCN/HVS staff with IT issues when required and liaise with EMIS over remote access or issues that arise with the system in the absence of the HVS Co-ordinator

Report back any issues staff

Minute taking for the Management team

Updating process protocols and ensuring they are up to date

Assist with the PCN Newsletter where required

Any other duties required by the PCN Manager

HVS (in the absence of the HVS coordinator)

Acting as a point of contact for the Home Visiting

To assist the HVS co-ordinator in the day-to-day operations of the HVS list and assist in the allocation of visits

Publication of service capacity programmes to the Practices in the PCN

Provide additional cover in times of Annual Leave

Provide daily support to the PCN Manager and HVS Co-ordinator and clinical team delivering the HVS

Ensure that all visits undertaken are recorded correctly in the medical records

Ensure patient feedback surveys are undertaken and uploaded as required by the service

Regular stock checks of stationery and medical supplies and assist with ordering accordingly

Printing of blood Forms in preparation for pro-active visits undertaken by the Home Visiting Team.

Assisting the HVS with referrals and workflow list. Including AccuRx text messaging to patients

To Monitor and undertake actions of the Generic inbox

To assist the co-ordinator with weekly care home ring rounds

To assist in the organising and booking of patients for Flu and Covid vaccinations
