Job summary
Fareham & Portchester PCN Ltd has an exciting new opening. We are seeking a highly motivated and experienced Advanced Clinical Lead Practitioner to join our dynamic health care team. The successful candidate will provide leadership for the existing team of Paramedics, Nurses, and General Practice Assistants, and play a crucial role in proving high-quality patient-centered care, contributing to the overall success of our healthcare services.
You will have the opportunity to work along side a multidisciplinary team supporting general practice and our patient population with the delivery of the home visiting service, and same day urgent care across the PCN. Lead and implement quality improvement and change, for health inequalities and patient access and demand.
Main duties of the job
You will be instrumental in providing clinical leadership for the existing team, supporting the Operations Manager with complex HR issues. Be involved in recruitment, appraisals, monthly documentations reviews, and clinical auditing. You will support the practices with acute home visit requests for care home and house bound patients, and same day care in practice. You will also provide support with pro-active home visits for patients with long term conditions
As Care home lead you will ensure the PCN is compliant with the requirements of the EHCH framework.
You will work with the PCN team on projects to reduce neighbourhood health inequalities.

Fareham & Portchester PCN (FPPCN) Comprises 4 practices. These are Fareham Centre Practice, Gudgeheath Lane Surgery, Westlands Medical Centre, and Portchester Practice. We have a patient population of approximately 44,500 with a diverse workforce of Paramedics, Nurses, ANP, General Practice Assistants, Care Co-ordinators, FCP Physio's, Social Prescribers and a Clinical Pharmacy team. We also pride ourselves on the excellent in-house supervision program we have been developing over the past 2 years to ensure maximum support and guidance from the GPs working alongside you. To
succeed, you will be a practicing advanced nurse practitioner/advanced
paramedic with experience in urgent care, chronic disease management and
leadership/management. As the team
leader for the home visiting service, you will provide clinical leadership for
the team and one to one supervision for the team members. You will work
with the management team to reach primary care targets, providing the clinical
opinion. As a clinician you will act autonomously within your professional
scope of practice providing triage, assessment and treatment for patients
ensuring clinical safety and excellence. This may be by phone, in a clinic, or
during a home visit.
Previous
Level 7 study is essential, together with evidence of working across the four
pillars of advanced practice. You will demonstrate a high standard of clinical
skills and have the ability to communicate, liaise and interact effectively
with multi professional teams across organisational boundaries. You will have
the support of senior colleagues when you encounter a limit to your competence.
Strong
organisation and time management skills are essential, as is the ability to prioritise
your workload. You will also have experience using various clinical IT systems,
in addition to basic IT literacy.
