Job summary
We are looking to recruit an enthusiastic, experienced clinical pharmacist to work within our growing team of pharmacists and pharmacy technicians. You will support all aspects of medicines management across our PCN including medication reviews, medicines reconciliation, the management of systems to ensure safe prescribing and offer support to the wider PCN team. You will also manage medication queries and repeat prescribing systems across our practices. In this role you will be supported by a Senior Clinical Pharmacist. You will work within your competence to deliver a service and manage the health and social care needs of patients within the GP practices included in Synergy Health PCN.
Main duties of the job
Key Duties
To reduce the medicines related workload on clinicians.
Increase safety and quality of prescribed medicines.
Provide support on medicines related issues to the PCN team.
Undertake medication reviews.
Be a point of contact for patients regarding medicines queries.
Core Duties
Undertake medication reviews with patients, implement prescribing changes (as an independent prescriber) and order relevant monitoring tests.
Chronic Disease Management (for example, managing hypertension/Type 2 Diabetes/Asthma/COPD patients, polypharmacy review).
Review of hospital correspondence including medicines reconciliation.
Act as a source of medicines information for all the PCN team and patients including suggesting potential interventions.
Support practices with the Quality and Outcomes Framework and disease register validation.
Support prescription management in line with practice and PCN policies.
To develop a good working relationship with community pharmacists and other local providers of healthcare.
To plan and organise own workload, including audit and project work.

Primary Care Networks (PCN) are local neighbourhoods where all health and social care services work together to deliver a better experience for their citizens.
Synergy Health PCN covers three practices in Gedling Borough, Jubilee Park Medical Partnership in Carlton with a branch site in Lowdham, West Oak on Westdale Lane, Mapperley and Trentside Medical Group in Netherfield and Carlton.
We are a standalone PCN with a lead practice. We have a population of 36,000 patients.
The PCN has care co-ordinators, social prescribing link workers, advanced nurse practitioner, first contact physiotherapists and clinical pharmacists. These roles will work closely together to develop and grow the PCN and improve patient care across our patient list.
Our vision is to build and develop collaborative working across member practices and stakeholders, to support and strengthen the delivery of Primary Care services and improve health outcomes for the local population we serve.
