About the role

You will work as part of a small team supporting:

  • Team Up housebound patients
  • The new Local Navigation Hub
  • A range of clinical services delivered by Swadlincote PCN

You will also serve as thesenior clinician on dutyfor theHome Visiting Service,Local Navigation Hub, and otherPCN-led clinical services,providing clinical leadership, mentorship, and oversight across these areas.

About us

Swadlincote PCN is a forward thinking PCN made up of 5 practices within the small geographical area of Swadlincote.

We cover a patient list size of approximately 60,000 patients.

You will be supported by the PCN Leadership Team, the Team Up Clinical and Operational Leads, and the Practice Teams, as well as the rest of the clinical and non-clinical team throughout the PCN.

KEY DUTIES TASKS AND RESPONSIBILITIES

Service Development, Delivery and Ways of Working

Work with the PCN Clinical Directors, PCN Manager, Team Up Operational Lead and other senior clinical leads within the PCN to develop and implement new care pathways for the service areas of acute home visiting, urgent community response, enhanced health in care homes, anticipatory care and other PCN led clinical services that supports the multi-agency and multi skilled team approach.

Act as an 'integrator' ensuring care is co-ordinated across the interface with Primary Care and other health and social care partners, including care homes, to ensure that service delivery is seamless and delivered in an integrated and holistic way.

In conjunction with the PCN Clinical Directors, PCN Manager, Team Up Operational Lead and other senior clinical leads within the PCN develop and implement robust systems and processes for the delivery of services and for ensuring effective clinical governance and quality improvement.

Support the teams in identifying and responding to safety and quality concerns, ensuring these are escalated as appropriate.

Support CST/MDT meetings across organisational boundaries to deliver appropriate patient centred care.

Promote a culture of self-managing teams who work to co-develop the service and address challenges from a multi organisational perspective.

Promote with the team appropriate ways of working consistent with a holistic Ageing Well service, such as holistic comprehensive assessment and enhanced advance planning.

Encourage the use of supportive, non-statutory services to promote a self-care approach.

Develop links with secondary care and other specialist services, to provide input and shared care working arrangements for complex individuals using the MDT approach.

Direct patient care

Providing medical assessment and management where needed on behalf of, or in conjunction with, team members.

Work with the Team Up operational lead and Home Visiting Service clinical lead(s) to develop and implement (with appropriate clinical protocol) the senior clinical triage function for the Home Visiting element of the service to determine the urgency and type of response according to clinical need.

Qualifications

Essential

  • A Doctor/Consultant on the relevant specialty register e.g. the GP performers list
  • Current registration with the GMC

Desirable

  • Royal College membership
  • Mentorship and Supervision Qualification or equivalent
  • experience (e.g. GP Trainer, DMP)
  • Qualification or equivalent experience in delivering
  • teaching programmes

Aptitude and personal Qualities

Essential

  • A collegial team spirit and to feel comfortable in a strongly multidisciplinary team environment
  • Able to take a consultative approach to their work,appropriately involving relevant people
  • Be able to put the needs of the team ahead of individual needs at busy times, periods of increased workloads(assessments, sickness absences or unplanned leave etc.). Respond in a flexible way to fluctuations in workload
  • Ability to work with limited supervision
  • Understanding of the pressures faced by Community healthcare teams
  • Willingness to contribute to and participate in a peer support group
