Job summary
To
provide a high-quality service to the patients of Swadlincote Primary Care
Network (PCN); diagnosing and treating patients while working within defined
procedures and protocols as laid down by senior clinicians within the PCN.
To
work as part of a clinical team, including Advanced Nurse Practitioners, a
Paramedic, Social Prescribers and Mental Health Practitioners based at Ragsdale
House at Woodville and across Practices in the PCN as and when required.
To
be responsible for seeing patients from the PCN base at Ragsdale House and across
the five practice sites as and when required and for visiting patients in their
place of residence as and when required, who present with a wide range of minor
illness, urgent and same day requests for treatment, providing care for the
presenting patient. Assess and triage patients, including same day triage, and
as appropriate provide definitive treatment, or make necessary referrals to other
members of the primary care team.
The
post holder will need to demonstrate safe, clinical decision-making and expert
care, including assessment and diagnostic skills. There will be the need to
prioritise and triage the needs of patients accordingly making any necessary
referrals for investigations in the appropriate manner.
Main duties of the job
Provide
assessment, diagnosis, and treatment at first point of contact by attending to
patients according to their needs either in the PCN base of Ragsdale House,
Woodville, in the PCNs Practices or patients homes.
Act as an
autonomous clinician, with the support and guidance from the GPs
Diagnose and
manage both acute and chronic conditions, integrating both drug and non-drug-based
treatment methods into a management plan.
Accurately triage
and prioritise patients, including in emergency situations, demonstrating the
use of a variety of techniques to elicit the history of an event/illness,
including past medical and drug history.
Recognise, assess,
and refer patients presenting with mental health needs
Telephone triage
as appropriate for clinics and home visits and provide access to GPs as
necessary.
Evaluate clinical
information from examination and history taking and initiate appropriate
treatment and / or referral, including admission to hospital if required.
Signpost
appropriately to other services in the identified healthcare pathway and to
refer patients to appropriate care settings, within locally agreed healthcare
pathways.
Maintain accurate
and systematic documentation of patient interactions and clinical care
provided, including adverse events.
Advise and
support members of the public on healthy lifestyles, health promotion and
disease prevention.
Provide clinical
advice to other members of the team where appropriate.
About us
We are an established forward thinking PCN with a team of five GP practices within the small demographic area of Swadlincote, holding a list size of approximately 58,000. Our PCN Team currently consists of an ANP, a Paramedic, 5 Nurse Associates, 2 Social Prescribers, 5 Health and Wellbeing Coaches, 2 Mental Health Practitioners, 5 Clinical Pharmacists and 7 Care Co-Ordinators along with a Leadership Team consisting of the Clinical Director, Deputy Clinical Director and PCN Manager.
We also have a well-established home visiting service team that work from the PCN site which includes GPs, Advanced Clinical Practitioners and Enhanced Care Practitioners.
You will be well supported by the Network Leadership Team as well as by the individual practice staff. We are a hardworking friendly and supportive team that puts patient need and staff wellbeing at the heart of everything we do.
We provide in house mentorship and training in order to allow staff to develop their skills, knowledge and confidence in a safe and supportive environment.
